MEMO OF NON-COMPLIANCE *Note: Not a Warning

- binibigay kapag may defects sa inspection ng inspectorate team na hindi narectify on
time
- shall explain in writing why you failed to comply within 5 days from the receipt of
memo
- photographs taken before, during and after completion of work should be attached
*should be taken at the same vantage point
- in short, SUBMISSION OF JUSTIFICATION

JUSTIFICATION
- contains explanation why DEO failed to comply, attached with docus such as Contract
and photos of remaining rectification work
- it should be submitted 5 days upon receipt of MEMO

EVALUATION
- Section Chief, Division Chief, and Bureau Director will evaluate the submitted
JUSTIFICATION
- photos shall have the same angle with identified landmark, background, reference
point and signature of MPP
- details on white board should be identical with the header of Picture Format

COMMON ISSUES OF PHOTOGRAPHS SUBMITTED BY DEOs

- no white board
- unidentical details on white board and header
- insufficient details written
- photos not taken on same vantage point
- different loc of before, during and after pics
- no before and/or during and/or after pics
- defect and/or quantity does not match the picture
- shoulders not properly compacted
- scattered grass in vegetation control
- improper disposal of spoils in after
- improper method in rectifying the noted defect

POINT SYSTEM
60% - (1) Inspection
40% - (2) Rectification
25% - (2.a) Validation
15% - (2.b) Justification

Procedure of Point System

1. Get the inspection quantities in from BOM-MPP-SOM (INS)
2. Is there any quantities to be deducted?
3. Compute for justification quantity per response time
4. Get 60% of the average weighted grade
5. Get the validation quantity from BOM-MPP-SUM (VAL)
6. Compute the response time 1 (no of days between the end date of inspection and
start of validation)
7. Compute for validation quantities per response time 1 (VAL/RT1)
8. Get the weighted grade of each code defect
9. Get 25% of the average weighted grade
10. Get the total quantity of additional rectification work per code defect based from
evaluation (JUS)
11. Compute for response time 2 (diff of the issued date of justification letter from the
end date of validation minus 5 days)
12. Compute for justification quantities per response time 2 (JUS/RT2)
13. Get the weighted grade of each code defect
14. Get 15% of the average weighted grade
15. Compute the final rating
