    List of NSTP Forms Required for Community Immersion

    For NSTP Student Facilitators, they must submit the following:

    1. FORM 1- Team Student Locator


    2. FORM 2- Students Pledge/Waiver
    3. FORM 3 -Consent of Parents/Guardians
    4. JURAT FORM
    5. MEDICAL CERTIFICATE (schedule for colleges/unit are provided, starting February 15,
    2023)
    6. TRAVEL ORDER issued by the Office of the President
    7. letter of request to the barangay to conduct for immersion
    8. acceptance letter of barangay
    9. community needs assessment
    10. Memorandum of Agreement (MOA) or Memorandum of Understanding
    11. Approved Proposal
