    PRACTICUM/INTERNSHIP WAIVER FORM

    This is to inform you that my child, Bonn Benedict M. Sia, a BSHRM student of ABE

    International College of Business and Economics would like to perform his/her

    practicum or immersion in Batchmate Inn which has the address of Brgy. Utap,

    Tacloban, City.

    I have given permission to Bonn Benedict M. Sia to render a remote or virtual

    practicum/internship in the above mentioned establishment. It is to my understanding

    that the school and the establishment will exercise due diligence to keep my child safe

    especially from CoViD-19. It is also to my understanding that my child and I are equally

    responsible for his/her safety. Should circumstances arise that are beyond the control of

    the school, it will be free from any legal responsibility.
