OB BAG TECHNIQUE – GUIDE

Materials:

 OB BAG
 Basin for water
 Paper lining (or manila paper)
 Plastic lining (plastic cover as alternative if
wala)
 Hand towel, soap dish
 White apron
 Alcohol lamp
 Test tubes na dalawa
 Urine sample
 Acetic acid
 Benedict’s solution
 Dropper
 Waste receptacle (pang dispose sa tabi ng work
area, kahit hanap nalang random container as
alternative)
 Paper and pen for documentation

1.Define OB BAG
2.Its RATIONALE

OB BAG: is also known as a PUBLIC HEALTH BAG where it


is a technique procedure that the nurse use during her
home visit when performing nursing procedures.

= It is also an essential and indispensable equipment


of the public health nurse we s/he has to carry along
when the s/he goes out home visiting. It contains basic
medications and articles which are necessary for giving
care.

RATIONALE: to render effective nursing care to clients


and members of the family during home visit.
STEPS/PROCEDURES

1. Identify client before explaining the procedure.


2.Place the bag on the table lined with paper lining,
clean side out. Put the bag's handles / strap beneath
the bag.

Rationale: to protect the bag from contamination.

3.Prepare a basin of water/ glass of water if faucet is


unavailable (place outside the work area)
4.Open the bag and take the plastic lining and spread
over work field/area. Paper lining must be clean side
out (folded part out)

Rationale: to prevent contamination from your work


field.

5.Take out hand towel, soap dish, apron and place them
at 1 corner of the work area. (beside the plastic
lining)

Rationale: to prepare hand washing.

6.Do the hand washing, wipe dry with towel, and leave
the plastic wrappers of the towel in a soap dish in
the bag.

Rationale: hand washing prevents possible infection


from healthcare provider (hcp) to the client.

7.Put on apron (right side out and wrong side decrease


touching the body, sliding the head into the next,
and neatly tie the strap at the back.
8.Put out things most needed for the specific case and
place at one corner of the work. Place bag outside of
work area and close the bag.

Rationale: to make them readily accessible

9.Proceed in performing the urine tests

[URING TEST FOR ALBUMIN]

1.Urine must be collected early morning before


breakfast and after external
2.Start to light the alcohol lamp
3.Prepare one test tube and fill with 2/3 or ¾ of the
urine.
4.Heat the upper portion only with urine. Fresh and
uncontaminated urine will reveal significant
findings.
5.When urine boils, observe for any changes of the
urine portion.

Results:

NO CHANGE: for albumin is an indication that (acetic


acid need not be added)

CHANGE: When cloudiness appears after boiling, (add 5


drops of acetic acid to the urine). The cloudiness is
due to the presence of phosphates.

6.Place test tube to the waste receptacle when adding


acetic acid

Rationale: to collect any waste of acetic acid if it


accidentally leaks.

7.Reboil the urine


8.When cloudiness disappears after boiling, urine is
negative (-) for albumin.
9.When cloudiness remains after boiling, the urine is
positive (+) for albumin.

URINE TEST ALBUMIN INTERPRETATION:

 Slightly cloudy = +2
 Moderately cloudy = +3
 Thick cloudy = +4

[URINE TEST FOR SUGAR]

1.Measure 5ml of Benedict solution in the medicine


glass, place solution on test tube.
2.Start to light the alcohol lamp
3.Put 8 drops of acetic acid to the test tube.
4.After 5 minutes of boiling, take note of any change
to the color of the solution. (Interpret correctly)
URINE TEST SUGAR INTERPRETATION: BLUE GREEN TRACES

 Yellow orange = +2
 Orange / brick red = +4

DONE!!!

*AFTER URINE TEST PROCEDURES

1. After completing nursing care, clean and sanitize


things use.

2. Do hand washing again to prevent contamination and


spread of infection to others.

3. Open the bag and return all things that were used in
their proper places after cleaning them.

4. Remove apron folding away from the body with soiled


side (folded inwards and clean side out). Place it in
the bag.

5. Fold plastic lining and place it in the bag and


close the bag.

6. Make post visit conference on matters relevant to


health care. (Take note of the date, time, and purpose
for follow up care)

7. Get the bag from the table, fold the paper lining,
insert inside the bag and and cover the bag.

8. EVALUATION & DOCUMENTATION (Record all relevant


findings)

9. Take note of environmental factors affecting the


client. (Conclude nurse-patient relationship)

10. Assess effectiveness of nursing care provided.
