Chief Complaint: 2 months missed menstrual period
Final Diagnosis: Ineffective cerebral tissue perfusion

DIET
INSTRUCT THE CLIENT TO:
● Eat foods that will help improve blood flow and tissue functions. (green leafy
vegetables such as cabbage and malabar spinach or also known as alugbati)
- 1 serving of cabbage and 2 cups of malabar spinach a day.

● Eat foods that will help boost oxygen level. (Fruits and vegetables; Avocado,
citrus drink like calamansi juice and water spinach or also known as kangkong)
- At least half of an Avocado after a meal.
- 2 cups of water spinach per day.
- At least 1 glass of calamansi juice after breakfast.

● Eat foods that are rich in protein. (Such as eggs, meat and fish)
- At least 1 egg (boiled egg) during breakfast.
- No more than 4 ounces of total meat per day.
- Consume between 8-12 ounces of fish per week.

● Eat 1 banana per meal if necessary.

● Drinking beverages that will help enhance oxygen level. (like blended moringa
leaves or typically known as dahon ng malunggay)
- Drink at least 1 glass of moringa leaves a day.
HYGIENE
● Set a daily routine - take a bath, brush teeth, dress appropriately. Good
hygiene starts your day off right and it helps you feel good about your
day-to-day living and creates order.
● Wash hands with soap especially after going to the toilet. Pregnant women are
vulnerable to infections.
● Brush the teeth lightly. During pregnancy, estrogen levels increase which can
cause swelling and increased sensitivity in gum tissues.
● When washing the perineal area, wipe from front to back. This will prevent the
spread of bacteria from the anus to the vagina. Wash your hands with hot
water and soap afterwards.

ACTIVITY
INSTRUCT THE CLIENT TO:
● Engage in light exercises (morning walks, light house work, stretchings)
● Sleep at least 6-8 hours a day.
● Have adequate rest periods (including naps, relaxing and rest between
activities).
● Practice meditation as a daily routine. (Meditate as follows: Find your space,
choose a time when your mind is calm, be comfortable, take deep breaths,
free your mind from unpleasant thoughts, focus, and feel the peacefulness
that surrounds you).
● Engage in yoga to improve circulation, muscle tone and flexibility. (Start doing
yoga in the second trimester, after about 14 weeks and consult with your
doctor before doing so).
● Create a pregnancy journal and write down your pregnancy experience and
notable moments. It keeps you organized, alert, and in tune with yourself and
the baby.
● Have a sleeping position of less than 30 degrees or avoid using 2 pillows when
lying down.
● Practice proper breathing (in and out) patterns.
● Avoid doing strenuous activities (Activity that involves a lot of energy such as;
running, aerobic dancing, ranking leaves or mowing lawn, lifting heavy
objects).
● Refrain from sexual intercourse if possible especially after 6 weeks of
pregnancy.
- In the first trimester, vaginal intercourse frequency should be from 3 to 9
times per month. However, in the second trimester, it should be ranged
from 2 to 7 times per month. In addition, the frequency of vaginal
intercourse in the third trimester should be from 1 to 5 times per month.
Also note that your husband should be the one who will exert more effort
for you to avoid getting exhausted given your low hemoglobin levels.
MEDICATION
● Ferrous Sulfate 325 mg, 1 tablet once a day before meals and taken orally
(with 1-2 hours intervals before lunch).
● Vitamin B12 1000 mcg, 1 tablet once a day before meals and taken orally
(preferably every morning before breakfast).

COMMUNITY SUPPORT
● Inform the client that Silay City Maternal Health Care Center provides prenatal
care and vaccinations.
● Advise the client to seek counsel from someone who has previously dealt with
a situation similar to her own.
● Since the technology is rampant, advise the client to visit the website of
“Women's Health”. It will provide pregnancy learnings about what you can do
before, during, and after pregnancy to give your baby a healthy start to life.

SPIRITUAL
● Motivate the client to undertake personal reflection or meditation. (Praying or
talking to God to avoid negative thoughts)
● Encourage the client’s family members to spend quality time together
especially during Sunday masses.
● Encourage the client to share her feelings or express her emotions.

FINANCIAL RESOURCES/NEEDS
● Instruct the client to apply for health insurance.
● Instruct the client to seek social worker assistance with hospitalization
bills.
● Ask the client if she is a 4Ps (Pantawid Pamilyang Pilipino Program)
member. (4Ps members receive assistance from the government with
zero bill upon discharge).
● Commend the client to seek assistance in Malasakit Center.
● Instruct the client to ask for cash assistance from the municipality
mayor or any available government assistance.

APPOINTMENT
INSTRUCT THE PATIENT TO:
● Have a follow-up check up with an Obstetrician-gynecologist, Dr. ABC on
August 25, 2022 at 10 am in the morning.
● Schedule an ultrasound check up with the obstetrician, Dr. EFG on
September 4, 2022 where the pregnancy is in 9 weeks gestation.
(Preferably in the morning, 10:30 am)
● Have a regular check up during the 1st trimester of pregnancy every
20th day of the month with Dr. ABC.
