QUESTION 1
(a) Question 1 as below.
i. Based on your understanding, define what is engineering control? (2Marks)
ii. Fig. 1 shows a newly bought bench-type utility grinding machine at the KFD Eng. Sdn. Bhd.
The machine used for miscellaneous grinding, and cleaning works. As an engineer,
determine four (4) engineering control methods to minimize the risks to the workers. Justify
your answers. (You may use sketches to support the explanations). (8Marks)

Answer
i. Engineering control can be defined as a physical modification to a process or equipment
to avoid the release of contaminants into the workplace. (Or any logical definition) 2marks
ii. Any engineering methods with elaboration how it can improve safety condition. 1mark for
suggestion, another mark for elaboration. Other methods are not acceptable. 8marks
(b) Figure 2 illustrates the decrease of sound level with distance, while Table 1 shows permissible
exposure limit (FMA-Act139-P.U. (A) 1/1989). An old stamping machine in KFD Eng. Sdn. Bhd.
produce loud noise of 120 dBA and operating non-stop during the working hour. A group of
Assembly-Section staffs are working at 32 meters from the machine. By ignoring the sound
reflection effect;
i. Estimate and justify the maximum allowable working hour for the Assembly-Section staffs
without hearing protection. (4 marks)
ii. Determine and justify the necessity to provide hearing protection for Assembly-Section
staffs to work in normal working hours at current condition. (3 marks)
iii. Identify four (4) control methods, other than the above, that can be applied for ensuring
safety and health of all workers in the company. (8 marks)

i. Distance = 32m, Noise level Reduction = 30 dB, Noise level at Worker A = 120 -30 =
90dB, Hence maximum allowable working hours = 8 hours 0 minutes.
ii. Action level for noise exposure is 85dB for 8 hours working, which is equivalent to
50% dosage of exposure. Hence, hearing protection must be provided.
iii. Other control methods: (Example):
• Powerful noise sources should be enclosed by structures which give
adequate airborne-sound insulation.
• Rooms where there are sound sources should be provided with ceiling
cladding which absorb the incident sound.
• Office areas should be separated from building elements where vibrating
equipment is installed by a joint of elastic material.
• Maintenance of machine by replacing with highly damped material such as
rubber caps, hammer head etc.
• Administration controls such as scheduling and providing frequent break to
minimize the exposure. etc
(c) A pressure vessel is a close container designed to hold gases or liquids at a pressure
substantially higher or lower than the ambient pressure. The operation of pressure vessel has
high potential of safety hazards that could lead to leaks and complete rupture.
i. Identify which pressure vessels that must be operated by a competent person or not.
(2 marks)
ii. Determine safety control measures that can be practiced by a company involved in
installation, inspection, maintenance, and operation of a pressure vessel at their premise.

ii. Available safety measures for pressure vessels (1 mark for a point, another 1mark for
elaboration)
Student can answer based on the “FACTORIES AND MACHINERY (NOTIFICATION,
CERTIFICATE OF FITNESS AND INSPECTION) REGULATIONS, 1970” or other
related regulations.
Content may include the following:
• Certificate of Fitness system.
• DOSH Inspector will inspect / test / issue certificate of fitness.
• Company will do cleaning and preparation for the inspection.
• The CF has validity period -> periodical inspection.
• The system can ensure safe use of the machinery even the company do not have the
expertise to do the inspection of the pressure vessel.
(d) Cycling was once a necessary way of transportation, but now it is seen as a modish,
environment-friendly hobby and a trend that is increasing in our country. However, according
to Bukit Aman Traffic Investigation and Enforcement Department, 137 accident cases involving
cyclists were recorded nationwide from January to October 2020, which 56 of them were fatal
accidents.
Recommend five (5) measures how risk of accident involving cyclist can be minimized.
Elaborate your answers clearly.

An open-ended question. Student should suggest the measure (1mark) and elaborate how the
suggestion can help in reducing accident (2marks). Example:
• Improve Visibility: Increasing the visibility through the use of, for example, reflective or
fluorescent clothing, reflectors, and light or bright helmets for cyclists could help drivers
detect these road users.
• Compulsory PPE-Bicycle helmets: In addition to protecting against head injuries when
used properly, helmets may also increase the visibility of cyclists.
• Educational campaign: Public awareness campaigns aimed at cyclists to increase
knowledge and to motivate changes in behavior.
• Technology: minimize the risk of crashes include crosswalk countdowns and adding
flashers to yield signs. In addition, vehicle design to help reduce the severity of crashes
including flexible bumpers that absorb more energy on impact.
• Expand funding and implementation of safe routes to school programs and safety
education programs. Etc.
QUESTION 2
Figure 3 illustrates a common fast-food restaurant layout. As a management trainee at a renowned
fast-food restaurant company, you are required to identify hazards, investigate the risks, and
propose the risk controls to be implemented at the premise and the operations.
Generate a HIRARC report by identifying FIVE (5) hazards. Analyze the risks and suggest
the risk controls.

On the 31st of October 2019, one fatal accident case was reported to the Department of
Occupational Safety and Health (DOSH). The accident has occurred at one of the factories in
Selangor. In this accident, a general worker died after the forklift that he was driving overturned
and crushed him. You are required to:
i. Discuss immediate action that you should perform when you received the information.
(5 marks)
ii. List 10 questions to uncover the causes of the accidents for the victim and the
witness/witnesses (5 marks)
iii. Determine the countermeasures for this accident. (5 marks)
(For the details of workplace environment, you can make your own assumptions if necessary)
i. When accidents and incidents occur, immediate action may be necessary to: (come up
with at least 5 immediate actions)
e) What was the load that he was carrying during the accident?
f) Was the load blocking his view?
g) Was the victim working hours according to the normal shift hours or extended?
h) Tell me what you did from the time you arrived at work?
i) Point exactly where were you during the accident occured?
j) What type of shoes were you wearing?

situated in the UiTM campus. (7 Marks)
