QUESTION 1

a) You were attending laboratory class at the material and foundry laboratories the faculty.
Determine five (5) common types of hazard that can be found in that laboratories by
providing the sources of these hazards. Recommend the methods on how to minimize the
risk.

b) Company TRZ use chemical agent for cleaning their steel components before undergone
the coating process. The process poses a high risk to the workers safety and health.
Propose five (5) engineering method on how to control the risk. Justify each answer
clearly.

c) In recent months, accident numbers involving transportation of your logistic company were
found increased which cause injuries and properties damaged. Recommend five (5)
measures to assure the safety of the vehicle and driver while they are in operation. Give
clear examples.

d) A pressure vessel is a container designed to hold gases or liquids at a high pressure. Most
serious hazards in pressure vessels is the potential for cracking, which can lead to leaks
and complete rupture.
i) Identify suitable techniques to detect a crack in pressure vessel. State the reason.

ii) Interpret the consequence of ignoring the existence of crack in pressure vessel
(10marks)

QUESTION 2

a) A worker operates a cross-cut saw machine as shown in Figure 1. His work includes setup
the machine parameters, change the disk cutter, loading and cutting the wood. He also
needs to repair and maintain the machine regularly.

Prepare a HIRARC report by identifying FIVE (5) hazards. Analyze the risks and suggest
the risk controls.

QUESTION 3

a) During maintenance operation of road light at Precinct 12, Putrajaya, one of your staff was
fall to the ground. You are required to:
i. Discuss immediate action that you should perform when received the information
(5marks)
ii. Give 10 questions to uncover the causes for victim and witness (5 marks)
iii. Determine the countermeasures for this accident. (5 marks)
(For the details of workplace environment, you can make your own assumption if
necessary)

QUESTION 4

a. In November 2017, numbers of companies in Penang are staring at losses up to hundred

million ringgits after their factories were covered by floodwater. The companies looking for
cheapest possible options to continue their operations.

If you are the personnel working with the authority department, formulate five (5)
recommendations as a guideline to ensure safety and health operations before operation
can be resumed. Justify how the cost can be minimized?
