AFFIDAVIT OF DISCREPANCY

            I, JOHN EDWARD DE LA ROSA PAREÑO, of legal age, Filipino,

(civil status), and a resident of ________________, after having been duly sworn
to in accordance with law, hereby depose and say:

1. That in my Certificate of Live Birth as recorded in the Office of the Civil

Registrar of Antipolo City, Rizal, Philippines, my middle name was
written as "de la Rosa". (A copy of my Certificate of Live Birth is hereto
appended as ANNEX “A” and forms an integral part hereof);
2. That during my childhood until high school age, I have been accustomed
to the use of the name “JOHN EDWARD R. PAREÑO” in all my
papers and documents, and been using the middle initial “R” for De la
Rosa, instead of “D” or “DR”;
3. That I attest that “JOHN EDWARD R. PAREÑO”, “JOHN
EDWARD D. PAREÑO”, and “JOHN EDWARD DR. PAREÑO”, is
one and the same person;
4. That from now on, I will be using my true and correct name, JOHN
EDWARD D. PAREÑO.
5. That I am executing this affidavit in order to attest to the truth of the
facts aforementioned, and for any legal purpose this may serve.
