Descriptive Phase (Clinical Script) – your narrative: What happened?
Today, I was in the medication administration room, to receive cases of that need an
injection, so I received a patient that need to injected by Declophen 1 ampule of 3 ml,
so I preparing the medication as I learned in the lap in the institute, so I prepared the
medication and starting to inject the medication and I did not aspirate first, so my
instructor give me a comment about that, I make a Kim’s model about this situation to
prevent failing the same situation again.

I finds an article that speaks about the importance of aspiration during an injection,
The nurses have to aspirate before giving an IM injection to ensure that the
medication is not inadvertently delivered into a vein. As the aspiration consists of
drawing back on the plunger once the needle has been inserted to see if any blood
returns into the syringe, as the clinical decision-making is influenced by many factors
and understanding these is necessary for changes in practice to be successful as this
happen to the nurses during the practice as they doing aspiration before injection to
get rid of the Fear associated with accidentally injecting into a vessel.
Critical / Emancipatory Phase: What would you do next time if you faced the same situation?
In the next time, I will do aspirate before injection and already did it with the next
patient at the same day with observation with my clinical instructor.
