Injury events investigation

For injury events, investigation must comply with the local legislation. Also, each site must stablish a local protocol
for notification, report and communication with the competent authorities that are entitled to.
Regardless the above, the Event Owner must create an investigation in the event management tool in Fiori from the
event notification, using the Timeline&5WHYs (default) or ICAM (subjected to the area Manager, advising by the
HSE team from their site/operation) investigation methodologies, ensuring to attach all the information and back-up
from the investigation in the event management tool in Fiori.

Significant events investigation

To determine the investigation methodology for significant events, type of facilitator, investigation leader and other
requirements, follow the criteria from the document “Our Requirements Event and Investigation Management for
HSEC and Other Events”.
It is not mandatory to develop an investigation for SE Hazard significant events. Nevertheless, it will be a prerogative
from the VP HSE from Minerals Americas, Asset President or General Manager (advise by their HSE Business
Partners) to develop an investigation if considered necessary, considering the event risk analysis, its repetitiveness
and/or its impact potential to the business purpose.
Significant events with actual severity level 4 or above must comply with “Our Requirements Event and Investigation
Management for HSEC and Other Events”. Also, to ensure confidentiality is maintained and controlled, should be
conducted under Legal Professional Privilege. If required, the Legal Group must be notified prior to the start the

agreed and later notified to the Action Owner through the event management tool in Fiori. If not possible, the Action
Owner can reject or request for change as soon as possible.

Owner) in the event management tool in Fiori, using the quality checklist available in the system. If the Evaluator
requires it or does not have the sufficient and necessary knowledge in investigation methodology, they can request
support from their HSE Business Partners to complete this evaluation.
It is highly recommended that the quality evaluation process is conducted the Investigation Reviewer in presence of
the investigation leader and facilitator, with the purpose to solve all doubts and facilitate the feedback process to

Reviewer (preferable an area Manager or General Manager), who can provide feedback on the action quality, request
clarifications and/or additional information to ensure the actions meets the expected quality, as it is implemented in
