Purpose
Define a process to manage Safety events to ensure compliance of BHP requirements defined in the documents “Our
Requirements Health, Safety, Environment and Community Reporting” and “Our Requirements Event and
Investigation Management for HSEC and Other Events” and define a method to spread learning of investigations, all
operations, projects and explorations of Minerals Americas.
Scope
All "in-scope" activities developed by employees or contractors in any of operations, projects or explorations of
Minerals Americas.
It excludes processes of notification and management of events according to the local legislation. For such cases the
site must establish its local protocol of notification, report and communication with the competent authorities that are
entitled to.
It excludes the management of events that were notified in 1SAP, previous to the roll-out of the event management
tool in Fiori. This cases must be managed using the previous 1SAP process, according to the last version of this
document.

Requirements
All Safety events must be assessed considering their actual and potential impact level according to the severity table
from the document “Our Requirements Risk Management”. Also, they must be classified considering their actual
impact level according to the following definitions1:
Injury: Event that resulted in an injury
Near Miss: An occurrence (something did happen) that could have potentially resulted in harm to people or the
environment
Hazard: Source or a situation with the potential for harm in terms of injury or ill-health, damage to the
environment or a combination of these
To define whether the event is in or out of the scope use the flow defined in the document “Our Requirements HSEC
Reporting - Master Data, Definitions and Interpretations".
Safety events management must be developed according to the requirements detailed in this document, the
instructions from the EMS Instructive and the document “Our Requirements Event and Investigation Management
for HSEC and Other Events”.
All Safety events must be reported in the event management tool in Fiori, according to the document “Our Requirements
Health, Safety, Environment and Community Reporting” and the diagram shown below:

Event review
Event notification must be reviewed in the event management tool in Fiori by the Event Reviewer, who must verify
all event details and impact, add extra or missing information, and identify the Event Owner, prior agreement with all
areas involved.
Only events that have already been reviewed and completed by the Event Reviewer will be considered for official
safety statistics and reporting from each Asset/site/operation/project, hence this process should occur as soon as
possible after the event occurrence.

Event Reviewer criteria in the event Management tool in Fiori are as follows:
Important:
Significant Events resulting in a fatality or recordable injury (medical treatment-AL2 or lost time injury-AL3) must be
notified immediately to the President of Minerals Americas and HSE Vice President, and following the “Our
Requirements Health, Safety, Environment and Community Reporting”.

The general process to initiate and conduct an investigation is shown in the flow below:
If a significant event must be investigated through the event Management tool in Fiori an investigation team, Event
Evaluator and other relevant Stakeholders must be identified. The leader and facilitator from significant events
investigations must have completed the official Investigation Methodology training, complying with the criteria defined
in the document “Our Requirements Event and Investigation Management for HSEC and Other Events”.

To ensure that each corrective action can be completed in time and scope, all corrective actions must be previously
agreed and later notified to the Action Owner through the event management tool in Fiori. If not possible, the Action
Owner can reject or request for change as soon as possible.

The general process for the review and completion of the investigation must be conducted according to the following
flow:
Investigation completion
All investigation must be completed (closed) in the system within the deadlines defined in “Our Requirements Event
and Investigation Management for HSEC and Other Events”, which can be extended prior with Event Reviewer
approval.

The general investigation review and completion process must be completed following the flow below:
Each Action Owner is responsible for the action to be completed (closed) in the system, verifying that the action has
been implemented and completed in scope, attaching all the minimum back-up information in the system to account
for its implementation.
All corrective actions from significant events SE Injury and significant events resulting in fatality must define an Action
Reviewer (preferable an area Manager or General Manager), who can provide feedback on the action quality, request
clarifications and/or additional information to ensure the actions meets the expected quality, as it is implemented in
scope and address the gaps identified in the investigation.
