CONTRACT OF SERVICE

I, JUAN DE LA CRUZ, hereinafter called the First Party, of legal age, single, Filipino
citizen, with business address at AAA Catering Service, Brgy. 1, Laoag City, Ilocos Norte, and
I, JUANA CRUZ, hereinafter called the Second Party, likewise of legal age, single, Filipino and
a resident of Brgy. 2, Laoag City, Ilocos Norte, hereby enter into this Contract of Service under
the following terms and conditions:

1. That the Second Party shall hire the First Party to provide catering
services on the Christmas Party which shall be held on December 15, 2017 at
the Second Party’s residence located at Brgy. 02, Laoag City, Ilocos Norte.

2. That the contract price shall be ONE HUNDRED THOUSAND

PESOS (P100,000), fifty percent (50%) of which shall be handed by the Second
Party to the First Party upon the signing of this contract and the remaining fifty
percent (50%) shall be paid on the day of the occasion;

3. That in case the Second party fails to pay the balance on the day of
the occasion; an additional ten percent (10%) per month shall be imposed on the
balance. Failure to settle the same within a period of one (1) month shall make
the whole balance due and demandable.

IN WITNESS WHEREOF we have hereunto affixed our respective signatures this 10th
day of January 2017, in Laoag City, Ilocos Norte.

ACKNOWLEDGEMENT

BEFORE ME, a Notary Public for and in the City of Laoag and in the Province of Ilocos
Norte, this 10th day of January 2017, personally came and appeared JUAN DE LA CRUZ and
JUANA CRUZ, with his Community Tax Certificate Number 123456789 issued on January 09,
2017 at Laoag City, Ilocos Norte and Community Tax Certificate No. 987654321 issued on
January 09, 2017 at Laoag City, Ilocos Norte, known to me and to me known to be the same

person who executed the foregoing Contract of Service, consisting of TWO (2) pages, which
they acknowledged before me as their free and voluntarily act and deed.
