AFFIDAVIT OF UNDERTAKING

I, Rommel Mempin Cinco, of legal age, single, Filipino, and a resident of

No. 29, Purok 9, Sitio Tulay na Patpat, Brgy. Basuit, San Ildefonso, Bulacan after
having sworn to an oath in accordance with the law, do hereby depose and state:

That I deposited to the Branch Clerk of Court of the MTC 2, Calamba,

Laguna, the total amount of TWO THOUSAND PESOS ONLY (P 2,000.00)
under the Official Receipt Number ______________________dated January 12,
2023 as cash bail bond for Criminal Case No. 64926-20 THEFT in Municipal
Trial Court, in Cities, City of Calamba, Laguna;

That I am aware of the conditions to which said bail bond is subject, to wit:

1. That said cash bail bond will be effective upon its approval and
remain in force at all stages of the case until its final termination,
unless the Court directs otherwise;
2. That I will appear before the proper court when so required by said
Court;
3. That IF I FAIL to appear at the trial without justification despite
due notice, the same will be construed as an express waiver of my
right to be present on the date specified in the notice, thus trial
may proceed in absentia;
4. That I appear before the court for Promulgation of Judgment and
execution thereof, and;
5. That IF I FAIL to perform any of these conditions above-stated,
the cash bond of P 2,000.00 posted for the accused will be
forfeited in favour of the government of the Republic of the
Philippines.
