RE: VACCINATION SCHEDULE

Astra Industries is conducting a company wide voluntary covid vaccination

program. The company is highly encouraging all it’s employees to participate in
this activity as a counter measure for the Covid-19 disease Transmission. The
Vaccination will be held on October 16,2021, 2:30-4:30 in the afternoon at the SM
seaside. The program is in accordance with the company’s health and safety policy
that prioritizes the stability and safety of all of it’s workers. This program is
conducted by the Astra Industries in association with the Department of Health.
All vaccination expenses are paid by the company itself.

All participating employees must be on time and must wear face mask(face shield
recommended) and observe social distancing throughout the process.
