1. A Trainee Abandoned - Abandoning a trainee can cause a major problem, not


having enough experience is enough reason why you should never abandoned a
trainee.

2. Poor Training - A shortage of trained pilots leaves little margin for error when
there are storms or other operational challenges.

3.Oxygen - Once the euphoria is over, hypoxia renders one unconscious and can
cause brain damage or death.

4. Silencing a Warning -Can cause plane crashes, being not careful enough when
flying can be a terrible mistake.

5. A Warning Ignored - WARNING indicates a potentially hazardous situation which,


if not avoided, could result in death or serious injury.

6. The Wrong Kind of Experience - They may lack experience and cannot handle
tough and pressuring decisions which can lead to disaster.

7. Deadly Distraction - the lack of ability to pay attention; lack of interest in the object
of attention; or the great intensity, novelty or attractiveness of something other than
the object of attention. Distractions come from both external sources, and internal
sources.

8.Underpaid and overworked Pilots - Difficulty concentrating on tasks resulting in


procedural mistakes, lapses in attention, inability to anticipate events, higher
toleration for risk, forgetfulness, and reduced decision-making ability.
