 SALE OF QUOTATION / BID DOCUMENT

A complete set of bidding documents may be purchased by prospective bidders on

payment of a non-refundable fee as indicated below in the form of a Demand Draft in favour
of Principal, S.C.B. Dental College & Hospital, Cuttack payable at Cuttack from any
Nationalised / Scheduled Bank at the office of the Principal, S.C.B. Dental College &
Hospital, Cuttack during office hours from 11 A.M. to 4 P.M. on all working days as
mentioned in the quotation document either in person or by post.

The Bidders may download the Quotation Documents directly from the WEBSITE
available at http://dmetorissa.gov.in & http://orissa.gov.in/alladvertisement. The Quotation
cost fee of Rs.500/- (inclusive of VAT, Non-refundable) by way of separate Demand Draft
drawn in favour Principal, S.C.B. Dental College & Hospital, Cuttack payable at Cuttack
should be enclosed along-with the Bid. The Bidders should specifically superscribe,
“DOWNLOADED FROM THE WEBSITE” on the top left corner of the outer envelope
containing the Bid. The Quotation cost fee and the EMD amount should be submitted
separately in separate demand drafts. In case of any bid amendment and clarification,
responsibility lies with the bidders to collect the same from the website or the office notice
board before last date of sale of quotation document and the Principal, S.C.B. Dental College
& Hospital, Cuttack shall have no responsibility for any delay / omission on part of the
bidder.

Price of bidding document Rs.500.00 (inclusive VAT @ 4%)

(Rupees Five hundred only)

The quotation paper will be rejected if the bidder changes any clause or
Annexure of the bid document downloaded from the website.

MATERIALS & SURGICAL ITEMS ON RATE CONTRACT BASIS FOR A
PERIOD OF ONE YEAR FROM THE DATE OF PUBLICATION OF
APPROVED LIST

1.1 Sealed quotations will be received by Dated 27.05.2016 upto 1:30 PM by the Principal,
S.C.B. Dental College & Hospital, Cuttack, Orissa for the purchase of Dental
Consumable Materials. Any quotation received after the due date & time will be
rejected / returned unopened to the sender. The quotation will be received through Regd.
Post / Courier services / Speed Post / by Hand.
1.2 The Sealed quotations submitted by the quotationers will be opened by the Principal,
S.C.B. Dental College & Hospital, Cuttack in his office at 3.00 P.M. on 27.05.2016.
The quotationers or their duly authorized representatives are allowed to be present
during the opening of the quotations if they so like.

A. Details of Dental Consumable Materials:

The list of Dental Consumable Materials & Surgical Items to be supplied as per
Annexure – I

B. Documents to be submitted alongwith Bids :

1. Attested photocopy of TIN Certificate.
2. Price schedule, which should contain price of the Dental Materials including of all
taxes & transportation charges.
3. The quotationer must submit earnest money deposit of 1% of the bid amount, i.e. Rs.
12,000/- (Rupees Twelve Thousand) only in favour of Principal, S.C.B. Dental
College & Hospital, Cuttack in shape of Bank Draft from a Scheduled Bank
payable at Cuttack. The EMD of the unsuccessful bidders will be returned back after
finalisation of Bid.

C. Performance Security:

a) The selected tenderer has to deposit 10% of the purchase order value only in
shape of Bank Draft as performance security within 15 (fifteen) days of receipt
of the purchase order.
 b) If the firm fails to supply in time or supplies not of standard quality item or
violates the tender terms and conditions the EMD & performance security will
be forfeited and the firm will be blacklisted for two years.

D. Quotation terms and conditions :

a) The selected firms will have to start supply within seven days and complete full
supply within 15 days from issue of purchase order.

b) The items should arrive at the Principal, S.C.B. Dental College & Hospital,
Cuttack in good condition.
c) The rates will remain valid for one year from the date of approval.

d) The undersigned has the right to increase or decrease the required quantity.

e) In the event of the date being declared as a holiday for purchaser’s Office, the
due date of submission of bids & opening of bids will be the following working
date & time.
f) The authority reserves the right to accept / reject any part thereof or all the bids
& without assigning any reason thereof.

E. Issue of purchase order:

a. The purchase order will be placed to the concerned firms, who will quality L1
category.

F. Payment:

100% payment will be made after receipt of stock entry certificates and submission of
performance security.
