INSTRUCTIONS:

1. Attested copy of the last academic certificates/degree result card/detail marks certificate form a grade
17 officer or above must accompany the application form.
2. Nomination/permissions from the Head of the Institution / Organization/ Government Department.
3. Four passport size recent photograph.
4. Two copies of National Identity Card.
5. Domicile Certificate except evening programs.
6. Nomination letter for admission from respective provincial government (including Azad Jammu and
Kashmir) against their reserved seats in case the candidate is domiciled in the province other than
Punjab.
7. Separate application is required for each discipline.
8. Incomplete forms and those received after due date and time shall not be entertained.
9. Provisionally selected candidates will download this form and submit the same after signature to the
office of the Graduate Studies along with other documents after submitting the admission fee.
