Congratulations! You have been awarded a scholarship from Lone Star College-Montgomery for the
2022-2023 academic year.

Scholarship: LSC-Montgomery Healthier U Endowment

Scholarship Amount: $1,000

Requirements: Must attend LSC-Montgomery or LSC-Conroe Center, minimum part-time student,

minimum 2.75 GPA, enrolled in disciplines related to Natural Sciences and Health Professions, and funds
may be used for all educational expenses. Students who have a high number of credit hours and are
failing SAP due to max time frame or those holding previous degrees or advanced degrees are eligible
for consideration. Healthier U scholarship committee must review applications and make a
recommendation(s) with scholarship(s) to be awarded for Spring semester each year.

As the recipient of this scholarship, the following must also be met:

1. Be enrolled in a minimum of 6 credit hours at Montgomery for the semester(s) in which you are being
awarded.

2. Must meet (SAP) Satisfactory Academic Progress.

3. Meet, the scholarship requirements listed above.

4. Type a personal thank you letter to your donor. Please use th,e template attached as a guide.

6. Mandatory attendance at the Donor/Recipient reception. You will receive a Save the Date card and an
Invitation before the event.

7. Acceptance of your scholarship is also your permission to release personal information to the donor.

Please submit your typed and signed thank you letter by January 30, 2023, ***NO EXCEPTIONS*** (If
additional time is needed, please contact me)

If the thank you letter does not meet the requirements or is not submitted by the deadline, the
scholarship will be awarded to another student.
Please note: For your award to be posted, your “Thank You” letter must be accepted. It should be typed
and emailed as a Word document to the Scholarship Advisor at Kimberly.Hudson@lonestar.edu

Finally, if you have received this scholarship and have decided to go to another college, please let me
know as soon as possible so that another student might be awarded this scholarship.
