Profession : SPORTS SCIENCE (EDUCATION)
What is OSH : Creating a healthy environment and safe for peoples and student in school.
Also prioritise rules in sport and activities weather in school, gym or playground.

TASK 2

1) Case 1 : Ahmad Zaki Sdn Bhd. 2022/09/21. Construction


In my opinion the manager should do the health test for each workers especially for
the teams that work at chemical haardous. Health checks maybe looks simple but can
give a big impact to company.

2) Case 2 : SA TEAM ENGINEERING SDN BHD 2022/09/20. Manufacturing


For this case, for me its comman mistake where the supervisor or chief in
manufacturing doesn’t provide the prosedurs for workes about activties and rules in
concrete mold modification in building area.

3) Case 3 : KEAN FOO SDN BHD. 2022/09/19. Manufacturing


For every company, every accidents that harm the workers should be report to the
DOSH as soon as possible. But unfortunately, Kean Foo Sdn Bhd doesn’t do the same
thing. Where death occurs in company and they are not report it.

TASK 3

9 Duties in SHO listed in regulation

1) To advise employer on the safety and health measuress.


2) To inspect and determine the safety of work place
3) To investigate any accident wich has happened in the work place
4) To addidt employer in organizing and implementing OSH programme
5) To become secretary to the committee.
6) To assist the committee in any enspection of the work place
7) To collect, analuze and maintain statistics on any accident, dengerous occurrence,
occupational poisoning and desease which have occurred at the work place.
8) To assist any officer in carrying out his duty under the Act.
9) To carry out other instruction made by the employer on any matters pertaining to
OSH

TASK 4

List of the duty of employer based on OSHA1994 and FMA 1967

1) Ensure safety, health and welfare at the workplace


2) Employ a competent person as Safety and Health Officer
3) Formulate safety and health policy
4) Minimize and prevent exposure of risk to the employees.
5) Notify any dengerous occurrences.
6) Establish safety and health committee (SHC)
7) Conult Safety and Health Committee in safety and health aspects
8) Provide adequate information, training, instruction and supervision.

TASK 5

List of the duty of employee based on OSHA1994 and FMA 1967

1. It shall be the duty of every employee while at work


I. To take reansonable care for safety health of himself.
II. To co-operate with his employer.
III. To wear or use at all time any protective equipment to his safety.
IV. To comply with any instruction or measure on occupational safety and health.

2. A person who contravenes the provisions of this section shall be guilty of an offence
and shall, on conviction, be liable to a fine not exceeding one thousan ringgit or to
imprisonment for a term not exceeding three months or to both.

1) Occupational Safety and Health (Employer’s safety and health general policy
statements) (Exception) Regulations 1995.
2) Occupational safety and health (control of industrial major accident hazard)
regulations 1996.
3) Occupational safety and health (safety and health committee ) regulations 1996.
4) Occupational safety and health (safety and health officer) order 1997
5) Occupational safety and health (prohibition of use of subtance) order 1999.

TASK 7

5 OSH related guideline

1) Be aware. Always allert to what is happening in our surroundings.


2) Maintain correct posture. Use both hands to lift or carry a heavy object.
3) Take brake regularly. If we feeling tired and burned out makes us less likely to be
aware of our surroundings and is common cause of workplace injuries.
4) Use equipment properly. For me, only use machine we are trained and authorized to
use.
5) Locate emergency exits. We should know where exits located and keep the path to
other workers clear.

1) The Systems Theoretic Accident Model and Process [STAMP] (Leveson, 2004)
Causal Analysis based on STAMP (CAST) is needed to ensure understanding of the
entire accident process and all systemic causal factors involved.
2) Energy Damage Model (Viner, 1991)
hazard is a source of potentially damaging energy, and an accident, injury or damage
may result from the loss of control of the energy when there is a failure of the hazard
control mechanism
3) Time Sequence Models (Benner, 1975)
In ‘pre-conditions’, there is the opportunity to prevent the event occurring. Where
there is some time between the event initiation and the event. ‘Occurrence’ offers a
warning of the impending existence of an event mechanism and the opportunity to
take steps to reduce the likelihood of the event while in ‘Consequence’ there is an
opportunity to influence the outcome and the exposed groups.
