● Formative Evaluation Plan


For my formative evaluation, I will interview a sixth-grade math specialist, and two 6th
grade math teachers. Both instructors and the math specialist plan lessons in PLC with
me and are amongst the few I am training to use more technology in their classrooms.
Below is a table of questions I will ask the student and instructors in the one-to-one
evaluations.

For the small group evaluation, I will ask for verbal feedback at the end of the lesson. I
will also take notes of how students are using the foldable throughout the lesson. This
lesson will be conducted during PLC with all 3 evaluators in 90 min.

One-to-One Evaluations

● One-to-One Evaluations
○ Description of who served as one-to-one evaluators and why they were chosen
● 6th Grade Math Teacher (A) - This instructor is my teammate and a first
year teacher. I chose her because she is eager to learn non traditional
ways to teach math as a new educator and as a person who needs extra
help with math concepts.

● 6th Grade Math Teacher (B) - This instructor is my teammate and is more
of a traditional teacher who has been teaching for 18 years. She is
wanting to incorporate more technology with students and looking to
find more ways of engaging them.

● Math Specialist - This instructor is a math lab teacher and she was
looking for a platform to engage students in more skill based learning

○ Length of the one-to-one meetings


10 min each after school

○ Description of the activities done during the one-to-one meetings

We went through all of the instructional materials from the perspective of the
instructor. I asked questions about the complexity of the goal steps and if
anything was easy to follow.

○ Remarks on how long it took the evaluators to complete the activities


It took them approximately 10-15 minutes to complete the activities because
they worked together in PLC

○ Results of the assessments that evaluators completed


The evaluators struggled through a few of the steps on their own and asked for
clarity from either me or the other instructors in the room to help complete the
task. All 3 were able to upload all of their students successfully.

○ Feedback received from the evaluators during these sessions


Do you believe uploading names using the spreadsheet is more efficient than typing one by

All instructors agreed that this process was more efficient as for as time and accuracy of
student names

Which part of the process was difficult to execute?


All instructors agreed that separating the names using delimiters was the most difficult
mainly because it was a new skill

Was there a particular skill taught that you can add to your technology toolbox?
All instructors agreed that learning delimiters was the a skill to add to their toolbox

i. Make better how-to steps for separating student first and last names

4. Small Group Evaluation


a. Description of how many learners were in the small group and how they
represent some of the subgroups within your target learner population (such as
achievement level, native language, learning preferences, age, etc.).

There were 6 instructors in the small group evaluation. The lesson took about 30
minutes during our math department meeting on our last in-service day. There
were three 7th grade math teachers and three 8th grade math teachers. The
teachers were eager to learn a faster more efficient way to input students in IXL.

b. The length of the small group evaluation session


i. This lesson took about 30 minutes.

c. Description of the activities that you did during small group evaluation session
i. All teachers had their laptops and followed the how-to materials while
asking questions if needed.

d. Remarks on how long it took the target learners to complete the activities
i. It took the instructors 15-20 minutes to complete the task

e. Results of the assessments that target learners completed


i. Instructors were able to upload all current students in an efficient
amount of time

f. Feedback received from the target learners during these sessions


i. The instructors wanted a more in depth explanation of delimiters and
which ones are the best to select in the separation process

g. Description of revisions that could be made to instruction based on the feedback


i. In the materials, give a definition of a delimeter and an example of what
each one would look like or do

h. Links to any questionnaires, documents, or other materials created specifically


for your small group evaluation
