MEMORANDUM OF AGREEMENT FOR PROJECT TADLASS-L.A.Y.A.T.

PARTNERSHIP

This Memorandum of Agreement (“MOA”) is entered into by and between LIBERTAD NATIONAL HIGH
SCHOOL, represented by SHARON S. DY, and VOLUNTARY GROUP, represented by (NAME), collectively
referred to as “Partners,” for the purpose of implementing a reading intervention program for students
in LIBERTAD NATIONAL HIGH SCHOOL.

Purpose: The purpose of this MOA is to establish a partnership between The LIBERTAD NATIONAL HIGH
SCHOOL and VOLUNTARY GROUP to implement a reading intervention program that will improve the
reading skills and academic performance of students in LIBERTAD NATIONAL HIGH SCHOOL.

WITNESSETH:

WHEREAS, The COVID-19 pandemic created a negative impact on education worldwide.

WHEREAS, This holds true to the reading literacy among Junior High School students of Libertad
National High School.

WHEREAS, to assess learners' reading skills, the school used the PHIL-IRI Tool and was conducted last
October 2022 as one of the activities for National Reading Month.

WHEREAS, the findings shows that there are 150 or 33% frustration readers in Libertad National High
School, which means that these junior high school students does not have adequate background level
for a particular word/s or cannot meet criteria for instructional level of accuracy, thus; these students
require extensive intervention from the teachers, parents, PTA officials and in the community in general
October 2022 as one of the activities for National Reading Month.

WHEREAS, to address this learning literacy gap, the school shall implement the Project LAYAT with a
fervent hope that this project will gradually develop reading ability and will somehow forge students
onward as instructional or even an independent reader in the future years to come.

Roles and Responsibilities

a. LIBERTAD NATIONAL HIGH SCHOOL agrees to:

1. Provide access to Libertad National High School and its students for the implementation of the
reading intervention program.
2. Provide a dedicated space for the program.
3. Provide any necessary materials and equipment for the program.
4. Designate a school administrator or teacher to oversee the program.
5. Provide certificate of recognition to volunteers.

b. VOLUNTARY GROUP agrees to:

1. Provide trained reading tutors to implement the program.


2. Develop a program curriculum that aligns with the school’s reading goals and objectives.
3. Provide ongoing training and support to tutors.
4. Collaborate with the school administrator to ensure the program’s success.

Scope of Work:

The reading intervention program will provide targeted reading instruction to students in grades 7-12
who are identified as needing additional support in reading. The program will be implemented during
the school day, and students will receive 1-2 hours individual or small-group reading sessions twice a
week. The program will use evidence-based reading strategies, including phonics, fluency, vocabulary,
and comprehension, to improve students’ reading skills.

Evaluation and Monitoring:

The partners will establish a system for monitoring and evaluating the program’s effectiveness. [Name of
Non-Profit Organization] will provide regular progress reports to Libertad National High School on
student achievement, tutor effectiveness, and overall program impact. Libertad National High School
will provide feedback and support to improve program implementation.

Amendments:

This MOA may be amended or modified by mutual written agreement of the partners.

Entire Agreement:

This MOA constitutes the entire agreement between the partners and supersedes all prior negotiations,
understandings, and agreements, whether oral or written.
