Minutes of the Opening and Evaluation of Bids for the KC-NCDDP Additional Financing project
of Barangay Mahayahay, Maigo, Lanao del Norte held at Barangay Hall on June 14, 2022, at
10:30 in the morning.

a) Ning-tindog si BAC Chairperson Desizen M. Colanag nagpahibalo ug nagsulti sa

iyahang committee, TWG ug uban sa Kalahi-CIDSS Maigo staff nga ang aktibidad
nga paka-buhaton karong buntaga mao ang Opening of Bids Documents sa
supplier/contractor nga naghulog sa ilaha Bid Docs requirement sa atong project nga
Setting-up of 1-unit Temporary Isolation Facility with two (2) Beds,
nagsugod ta sa oras sa 10:30 sa buntag.

b) Paga sugdan nato ang atoang aktibidad pina-agi sa pag-ampo, led by Barangay
treasurer Bernardita Miñao. Pagka-human sa ampo nitawag ug attendance si BAC
Secretariat Susan R. Sereño sa mga BAC members ug kay present man silang tanan,
mao ni declara siya nga may QUOROM.

c) Sa wala pa sugdi ang pag-abli sa bid box gipasulti atong BAC Chairperson ang
amahan sa Barangay si Kap. Jerry M. Culanag diin nag-pasalamat sa mga
nanambong labi na sa mga BAC Committee para malampuson ang pagka-himuon
nga pag-abli sa bid box. Pagka-human ug istorya ni Kapitan gihatag niya ug balik ang
katunganan kay BAC Chairperson.

d) Ni-ingon si BAC Chairperson sa oras sa alas 10:35 ubanan sa iyaha BAC member
ilaha gi-open ang bid box. Ni ingon si BAC Chairperson nga naa’y duha (2) ka bidders
ang ning-hulog mao kini sila,

e) Ni-ingon si BAC Chairperson ang envelope sa duha (2) ka bidders kay naka sealed.
Ang unang ablihan nato nga envelope mao ang ilaha eligibility ug technical
documents. Ang ilahang ika-duha nga envelope mao ang financial documents kay
atoa sang ipa-daplin. Ang basihan sa eligibility ug technical documents basi sa R.A.
9184 mao ang,
 Phil-GEPS Registration Certificate
 SEC, DTI or CDA Certificate
 Mayor’s or Business Permit
  BIR Tax clearance
 Statement of all ongoing contracts (Gov’t. or Private)
 Single Largest Completed Contract (SLCC)

Ang sulod sa 2nd envelope mao ang financial documents mao ang musunod,

f) Si BAC Chairperson nag-ingon nga si Eight King’s Construction and

Enterprises ug Greenergy Development Corporation, ang duha (2) ka
bidders kay complete ang ilahang 1st envelope mao ang eligibility ug technical
documents.

g) Karon sa oras sa 11:05 sa buntag, ni-ingon ni BAC Chairperson Desizen M. Colanag

nga nahuman na ang Opening sa mga Bid Documents sa atoang mga bidders.

h) Ni ingon pod si BAC Chairperson karon sa parehas nga oras sa 11:05 sa buntag atong
isunod ug sugdan ang pag-evaluate sa atong duha (2) ka bidders. Diri nato magamit
ang maong checklist nga atong gihimo sa manila paper nga naa’y “pass or fail”.
Ang una natong e-evaluate mao si Eight King’s Construction and Enterprises
kay mao man atong una gi-ablihan kaganina.

daog sa atong gipahigayon nga bidding sa atong project nga Setting-up of 1-unit
Temporary Isolation Facility with two (2) Beds, mao sila atong e Post
 Qualification karong umaabot nga June 24, 2022 (Friday) didto sa ilahang opisina sa
Cagayan de Oro City.

m) Karon nga wala nata’y lain pang hisgutan ug angay nga klarohon, ato nang e-
adjourned atong opening ug evaluation sa oras sa alas 12:55 sa hapon, gisigundahan
ni BAC Member Danilo B. Ronolo.
