Action Plan
Create a healthy self-management action planning using your self- management steps as a
guide. The plan should be designed to support your family and friends in coping with covid-
19. An example is already done for you although not directed towards covid-19 it should serve
as a guide in what is expected to be in your plan.  

Group leaders please ensure to make contact with your team, you may do so using Google
Meet, Whatsapp or any other platform possible FACE-TO-FACE Meetings are NOT allowed.
Although you are planning collaboratively, each person still has to turn in their own copy of
the completed plan. A rubric is attached in Google Classroom you may use it as a guide.
