III: PRESENTING PROBLEM
The client verbally stated that she cannot fully provide the needs of her four children
and her senior citizen mother.

IV: HISTORY OF THE PROBLEM

The client and her family are staying in Nambucayan, Tabuk City, Kalinga. Her
children are currently entering school at Nambucayan Elementary school and
Nambucayan National High School. She and her husband are farmers. While at work,
the mother of the client who is a senior citizen with hypertension issues cook for lunch
for the children. The couple plant grains, corn, and banana for their food, also, after they
gather some of their plants like vegetables and fruits, they sell it at the public market
and buy something useful for their kitchen like oil, sugar, and other needs in their daily
life. Sometimes they exchange their products to fish and meat for “ulam”. Even though
the couple are doing their best, the client admitted that they cannot fully provide the
needs of her children and her senior citizen mother’s medication for her hypertension.

V: BACKGROUND
A. CLIENT
The client is from Balbalan, Kalinga, she is the third child in a sibling of six
and she finished high school at Luna, Apayao in 2006. She has a small smooth
tan face, thin, black, short hair with brown skin. She is 4’2 and her weight is 47
kilograms. The client is very approachable, jolly, and responds very nicely during
the interview.

B. FAMILY
The client and her husband met at Luna Apayao and was wed on May 19,
2007. As of now, the client and her family are currently residing at Nambucayan,
Tabuk City, Kalinga in a small house made of concrete and woods with her
mother who is a senior citizen with hypertension issues and their four children.
Their children are attending public schools whereas the eldest, Arnalyn is a high
school student and her three other siblings Arealyn, Arcelyn, and Arjon are
elementary.
The family is a well oriented with one another and are happy together. The
mother of the client functions as the nanny of the children as she takes good care
of them when their parents go to the field or other places to work. She cooks and
look after them making sure that they are well fed before going to school and
also when they come back home. The client and her husband are thankful of
their mother but at the same time worried because of the fear of their absence
when she might have hypertension. Overall, the family members have healthy
relationship with one another.

C. COMMUNITY
Barangay Nambucayan is a beautiful barangay, surrounded by a clean
environment. It is rich of agricultural resources like corn, grains, bananas, and
various vegetables. The people are hospitable and composed of different tribes,
culture, and belief. It has a small barangay hall and promotes proper waste
segregation. It has public elementary and a high school level school. It also has
a health center. Water source comes from springs and pump wells. The common
livelihood of the community is farming rice, corn, and banana production.

D. ECONOMIC
The client and her husband are the only source of finance for the family.
They are doing the best to provide for their family making sure that their mother
and children will not starve, and all their basic necessities are met. the client is
member of 4P’s, and both works as farmers. As of now, the income from their

products is not enough for the expenses for food, educational needs of their
children and the medical expenses of her senior citizen mother who is suffering
hypertension.

VI: ASSESSMENT
The client is a woman who is resilient and has a strong family moral support but
is in need of financial security for the basic needs of her family and the maintenance for
her mother’s hypertension.

VIII: RECOMMENDATION
Considering the data gathered and with the approval of the client, the worker
therefore recommends the client to the International Association for Transformation
office to seek for educational sponsorship and for the registration of the client’s mother
for the senior citizen’s social pension. I also recommend the client to approach the
barangay office to inquire for the organization of farmers.

The client said that she cannot fully provide the needs of her child’s educational
expenses.

IV: HISTORY OF THE PROBLEM

The client is a solo parent who is singlehandedly raising her last child and
supporting her education. Since, the death of her husband, she has no one else to help
her in their daily expenses, the client doubled her effort to have more money and food.
Aside from a small backyard garden, she employed herself to any available work like
cleaning fields and planting/harvesting corn. She stated that it was hard but she said
she did not know any other works because she did graduate and don’t have a diploma.
She said that as her child is going to be a college student soon, the expenses will even
more be heavier than it already is. She has no other source of income but to rely on
available work.

V: BACKGROUND INFORMATION
A. CLIENT
The client is a 46-year-old female with an average height of about 5
feet, and weights about 50-55 kilos. She has long black and white hair, a
blown facial complexion with little wrinkles. She is kind and have a strong
but gentle tone and is serious when having conversation. She is also
smart and industrious.

B. FAMILY
Mrs. Domingo was 20 years old when she met her husband who
was from Santiago, Isabela at a wedding of her cousin. At the time, she
was still studying college when her husband started courting her. A few
months later, she got pregnant and because of this she stopped her
education and focus on taking care of herself for her baby. The couple
was then wed and had their own family. They then had three babies more.
After 8 years of their happy relationship, a very sad event took place. That
was when the husband of the client, due to an unprecedented accident
while riding a motor bike, he hit a dog while doing a sharp curve and
crashed leading to his death. The client was devastated because of the
event but had to be strong for her now fatherless children. She then tried
her best to raise them well make sure they go to school but the first three
of her children do not have interest on education, instead, they prefer to
go in the forest to catch wild animals like cockerels, lizards, and etc. to
take home as “ulam.” The client even though explaining the importance of
education can not persuade them to continue their education. Only the last
child continued her education. Years later, her three sons got married one
by one and went to the place of their wives living the client with their last
sibling.

C. COMMUNITY
The community is peaceful because there are only few crimes present as
according to the client. The people are respectful to visitors and to one
another. The houses in the community are made of wood and cement.
Theirs is also electricity, and the source of water is from mountain springs.
There are different sources of income for the community like farming,
stores, job employments, and other businesses. The community
participates on festivals or celebrations like barangay fiestas and religious
activities.

D. ECONOMIC STATUS
The client and her last child lives in a bungalow made of wood and
cement. They have access to electricity, they use gas tank for cooking,
and have their own toilet. They have a backyard garden where they can
plant vegetables. The client works as a daily wage earner often as a
farm/field worker for income to supply for their food and other basic needs.

The other three children of the client would visit occasionally to check on
them and give them cash to add with her income.

V: ASSESSMENT
The client is a single mother who is supporting her child’s education but can not
fully provide for her because she does not have a stable source of income.

VI. INTERVENTION PLAN

III: PRESENTING PROBLEM


The client explained that he needs assistance and recognition as a ‘person with
disability’ because his eyesight is very poor.
IV: HISTORY OF THE PROBLEM
The client said that he was only a child when he experienced poor eyesight and
weak hearing. He was further explained that he was more or less six years old when he
became aware of his condition and that he was checked up by doctors and they
explained to his father that some veins connected to his eyes were severed causing to
his loss of seeing and hearing. His last remembered checkup was way back when he
was still a grade 4 elementary student. Until then, he was only given glasses to help but
according to him did no good effect so he did not use them. He also gave up on taking
his maintenance medication because he cannot afford them. Until now, the client is still
almost blind but he was never officially recognized as a person with disability and
therefore did not enjoy the privilege of one. Because of his eye condition, he works as a
washer of the clothes of neighbors to be able to help on the daily expenses of his family.

V: BACKGROUND INFORMATION
A: THE CLIENT
The client is a 31-year-old male with a lean body figure with 5’5 height and 55
kilos weight. He has a light brown skin with black hair, long enough to bundle. He has
poor eyesight and can not hear well but he is quite a happy person, and he likes to joke.
Even with his current condition, he believes that God has a great plan for him that is
why he is still optimistic of a better tomorrow for him.
B: FAMILY
The parents of the client were blessed with 8 children including the client himself.
According to the father of the client, they were a happy family and that they have great
plans for their children but everything changed when his wife died. The mother of the
family died due to pancreatic cancer. Since then, the father did his best to support his
children by employing himself to carpentry, masonry and other jobs for daily wage that
other people could offer. Over time, three of the client’s brothers left the house to go find
work in the cities. Currently, the family only have each other for support doing what jobs
they can do to help in the expenses.
C: COMMUNITY
The client lives in a community who are populated by mostly of people from the
tribe of Lubuagan. The houses are made of cement and wood, and they are hospitable
to visitors. The community is living through farming, sari-sari stores, daily wage earners,
and office employees. The community is caring to one another as evidence to the
neighbor of the client who let them lend a part of his lot for them to use as a garden.
D: ECONOMIC
The family is living in a bungalow house made from wood and cement. The
client’s father is a gardener who plants petchay and other vegetable on a lot where a
neighbor let him use for the garden. He is the main source of financial support for the
family while his mother takes good care of the household chores. The client helps on
the expenses through washing of neighbor’s clothes for a wage of 300.00 Php per

service. The other siblings of the client also are working as daily wage earners because
none of them finished their education.

VI: ASSESSMENT
The client is a person with disability who is not officially recognized and have no
regular job for earning an income to help himself. Therefore, he needs assistance from
the office of the PDW office.

Self-Reflection

To be an intern in the role of a social worker in an agency is a big privilege for me


as a student. It has been a fruitful season for me as I have learned a lot of lessons as
well as applied them into their proper application. In this paper I well discuss two of such
lessons of which made a great impact on my perception of working as a professional
social worker. Firstly is the importance of being properly equipped with full knowledge of
social work because it allows the social worker to make right decisions, find all possible
choices and implications for clients’ concerns, and to utilize a range of approaches as
well as tools so as to adjust to the case’s scenarios. With a proper knowledge on the
field of social work, social workers are more effective and efficient.
The second lesson is the value of professional ethics in the office or the field of
work. In an agency where clients are emotionally, physically, or psychologically in
distress, it is important to always uphold professionalism never to let rough unnecessary
emotions out because it will cause complications in the process of helping them. There
is also the proper professional relationship with colleagues whereas as along as one is
in the office, it is good to maintain a healthy professional relationship and never to
include personal ones as it affects office productivity negatively.
There are values that one must uphold when being a professional social worker
and these two are part of those values that appealed to me deeply. In the next
internship, I hope for more lessons like these ones.

Today was the orientation for the on the job training held at the school campus.
We were instructed by our instructors and guided us for the coming OJT. The
instructors described possible scenarios inside the agencies that we will be entering and
reminded us to use what we have learned for the past semesters.

SEPTEMBER 1-2, 2022


In our first day in the office, we were trained to do emergency first aiding where
we learned how to properly first aid different injuries like burns, cuts, and choking. On
the second day, we learned how to bandage injuries and how to carry the patient with
the use of the spine board. We enjoyed doing the activities and learning from it.
SEPTEMBER 6, 2022
I entered the office at 8 O’clock in the morning and stayed at the lobby to assist
clients who are asking for blood service. I observed that most of the clients are seeking
blood due to dialysis. I was sad because there are many people suffering with this kind
of sickness.
SEPTEMBER 8, 2022
Today, we were trained to operate on the hotmeals on wheels program by
teaching us food safety and the use of the food truck. We spent the morning on the
lectures and training and in the afternoon, we went to Kalinga Provincial Hospital to
provide hot meals for the patients and the workers. We served until it was evening and
went home afterwards.
SEPTEMBER 9, 2022
The other volunteers and the agency supervisor went to Lubuagan to provide hot
meals and train the high school students on how to first aid. Some of the interns
assisted them and I stayed at the lobby again today.
SEPTEMBER 13, 2022
I was not assigned to do specific works so I helped assist clients and inquirers in
the lobby with my co-intern, Ms. Rovelyn Lardizabal.
SEPTEMBER 15, 2022
Today, we had a seminar on welfare desk management were we learned about
the proper approach on clients who approaches the workers for assistance. I learned
that it is important to always respond with a kind tone and not high tones because it will
affect the client. We also learned about mobile blood donation and the role of volunteers
who are not medical students or graduates.
SEPTEMBER 16, 2022
In continuation from yesterday’s seminar, we had a seminar today about HIV
prevention. I did not know that there are more people who are dying because of this

sickness than other sickness. After the seminar, I continued my duty until 5 and went
home.
SEPTEMBER 20, 2022
I was stationed at the lobby today and I think I am improving because of
constantly speaking with clients. It helps me develop my social skills to be more
effective on helping other people.
SEPTEMBER 22, 2022
Today is the Kabataan Fair and the Red Cross was invited to participate in the
event. So we went and set up our booth. We teach students basic first aiding and we
give children child friendly space. We assisted them and stayed with them while they
enjoy drawing with coloured chalks on the park.
SEPTEMBER 23, 2022
We were scheduled on the kabataan fair in the afternoon so I went to the office
and stationed there until lunch. I then proceeded to the kabataan fair in the afternoon to
help out. We conducted psychological first aiding to students who are in need of people
to talk to about their problems.
SEPTEMBER 27, 29, & 30, 2022
I was at the lobby the in the past three duties. It was the same scenario every
day because I just entertain inquirers and clients and refer them to the blood service
when they are in need of blood.
OCTOBER 3, 2022
I interviewed one of my clients today for the case work. It was not that hard
because I was used to speaking with clients because of my experience in the Red
Cross lobby. He was living in Lubuagan so I had to travel to his hometown for the
interview. I arrived late at 5:30 in the evening today.
OCTOBER 4, 2022
Back to the office again today and there was nothing to take note of.
OCTOBER 6, 2022
I only logged in half day today and helped on encoding the list of volunteers from
Rizal, Kalinga.
OCTOBER 7, 2022
Today, a client insisted to avail his insurance because of his motorcycle accident
but because he have no license for driving, he cannot avail it. He was insistent but we
patiently explained everything to him until he finally calms down and explained that he
needs the insurance money for his medication.
OCTOBER 11, 2022
There are many clients today as well as blood donors so we helped facilitate and
accommodate them as the medtech could not do it all by herself. It is a good thing that
we were oriented when it comes to releasing blood and of course how to operate on a
welfare desk.

Today was just like my last duty, full of clients and donors. So we again helped
the medtech on accommodating and assisting the clients. I could not accommodate
them all in the lobby so I asked the help of one of my fellow interns.

I only attended half day again today because I have to attend to one of my
babies who is having a fever.
OCTOBER 18, 2022
After a long weekend, I got into my duty and today, I helped distribute and collect
donation cans from all big establishments and businesses like bayle, Total, and etc.
after collecting the filled cans, we stored them in the office to be counted by the next
duty.
OCTOBER 20&21, 2022
A client came today to ask for blood but she can not pay for the screening fee so
I referred her to the MedTech. She then referred the client to the Philippine Charity
Sweepstakes Office for help. She availed the blood in the condition that she must pay
the screening fee later. On the next day, she came back with the voucher of the PCSO
for the blood she availed.

I helped a senior citizen who is in need of blood for his son’s dialysis.
OCTOBER 27, 2022
In preparation for the First Aid Station for the Undas, we had our meeting today.
We were divided into two groups to be assigned to Nambaran cemetery and Bulanao
cemetery. We also discussed about the job for each member of the team.
OCTOBER 28, 2022
We prepared the possible materials to be used un the Undas Operation like the
tents, first aid kits, thermometer, and etc. after then, I stationed at the lobby as usual.
NOVEMBER 1, 2022
Today is the start of the Undas operation so we were deployed early. It was not
as expected because only few people ever approached the first aid booth. Even so, we
remained on standby because accidents can happen anytime. In the end of the day, we
only served a few individuals who want to check their blood pressure and temperature.
MONTH OF NOVEMBER
in this month, all of my duty was on the lobby where Ms. Lardizabal and me
assist clients and inquirers who came to the office. We helped out clients who are
problematic on by applying PsychoSocial First Aid. We also helped preparing for the
annual Assemply and board meeting.
MONTH OF DECEMBER
We participated and assisted during the Red Cross Kalinga Chapter assembly
and board election on December 2. After that, we coordinated with the members for the
groupwork and we finalized our case studies for the case conference. I could not attend
to the case conference because there are many presenters and we only presented for a
day.

LEARNING INSIGHTS

As an intern, I discovered it’s essential to be enthusiastic and open to learning


new skills, asking for more work and being curious to learn and ask questions. This
attitude will show that I enjoy being part of the team and that I keen to help. Having
curiosity and enthusiasm also means that, as an intern, you get a lot out of what you’re
doing, which opens lots of opportunities. A great opportunities and a great experience
as one of the Social work Trainee is the most unforgettable moment in my whole life,
imagine I’ve done a lot of experiences, such as trainings and other activities inside and
outside the Agency.
Working in a professional setting for the first time can be difficult to get used to.
But it is the best way to learn how to navigate the working world through real-life, hands-
on experience.one of the most valuable skills I gain from an internship is the ability to
speak with people in a professional setting. Discussions with bosses or coworkers are
different from discussions with lecturers or fellow students, it is never been easy to
become an Intern but I choose to remain strong in finishing my hours than to dropped it
instead.
Philippine Red Cross Kalinga Chapter thought me a many lesson and new skills
that will be one of my instrument to fulfill my dreams in life. I’ve learned the best and
proper way how to communicate whatever clients that came into the Agency and
through Call as well. I will be forever grateful and thankful to my supervisor and the staff
who are my mentors and the one who teach me how to be a professional one someday.
I never regret in choosing that Agency as for my deployment for my first semester,
Learning new things from them is very helpful to me as an Intern for this year.
