TRAVEL WAIVER FORM

This is an agreement between 2k Princess Travel and Tours and

__RIZALDE GALLARDE ATENDIDO for the latter’s Tour package in November 07-11, 2022.

Services Provided

The Company offers travel-related services including the following:

Services Description
Manila-Thailand November 07 , 2022
Airfare via Cebu Pacific with or without 20 kgs
baggage
Roundtrip Airfare
Thailand-Manila November 11 , 2022 Airfare
via Cebu Pacific wit or without 20 kgs baggage

Hotel Accommodation Accommodation from November 07-11 , 2022

Travel Insurance Travel Insurance coverage with Covid 19 from

November 2022

The Travel Agency will offer only the above-listed services to the consumer clients as part of
travel packages pursuant to the terms of this travel agency agreement.

In return, the Client agrees to pay the Travel Agency on the services provided.

Travel Agency Responsibilities

The Travel Agency will offer items listed in this agreement to businesses and consumers as
part of travel packages.

The Travel Agency agrees to offer such items at prices approved by the Client.

Disclaimer
The Travel Agency will only be responsible for the services rendered above on the
inclusive dates.

Any activities and liabilities incurred by the consumer clients will not be of the Travel Agency’s
responsibility.

Insurance

The consumer clients' insurance is based on the third-party providers’ coverage.

Term

This Travel Agency Agreement shall be effective upon confirmation of booking and/or when
payment is received.

Payment is non-refundable.

All travel documents are provided by the clients solely for their use including purposes
that may not be outlined in the travel agreement.

This Travel Agency Agreement shall remain in effect for the coverage period listed only in
this agreement.

Termination

If any terms or conditions contained within this agreement are breached, the responding party
shall have the opportunity to terminate this agreement.

Upon the termination of this travel agency agreement, payment is non- refundable.

Acceptance

The Parties hereby acknowledge the receipt and understanding of all the terms and conditions
as outlined in this travel agency agreement.
