I, Shiela G. Gagote from this date March 20, 2023 is transferring the payment obligations of my
Unit (BLOCK 46, LOT 22) to my mother, Melva G. Gagote. Therefore, I am requesting you to
change the ownership of my Unit on Bella Vita Cabanatuan City to the present owner’s name
because I cannot update my payment due to financial reasons.

It is not possible for me to meet you in person regarding this matter; for that, please accept my
sincere apology.

Therefore, I give me next to kin Melva G. Gagote complete authority to take and make any
decision in this matter. And also give her the full authority to sign all the relevant documents
that are related to the ownership transfer and payment transfer procedure on her behalf of me.

I do take full responsibility for this above-said request and I hope you will oblige me by
considering my request.
