LEVEL II FIELDWORK EXPERIENCE
Level II at Encompass Health Rehab Hospital-Chattanooga TN (1/9/23-
3/31/23)
· Inpatient rehabilitation- At this fieldwork site I performed
evaluations, created and implemented treatment plans and plans of
care, and completed discharge summaries. This site had Center of
Excellence awards in stroke, amputation, hip fracture, and were
working on brain injury, so I was able to work hands on frequently
with these populations. I documented using an electronic health
record system.

Level II St. Francis Hospital- Bartlett TN (9/19/22-12/9/22)


· Inpatient acute- At this fieldwork site I frequently performed
evaluations specifically in the ICU and orthopedic, created and
implemented treatment plans and plans of care. I cotreated with
speech and physical therapy. I made recommendations for next
level of care following discharge from the hospital and documented
using an electronic health record system.

individuals without housing.
