1. INTODUCTION

Oil & Gas Company limited owns and operates the KUNAR re-construction crude storage Tank
Project in the Sindh near Hyderabad of Pakistan. The project covers Dismantling of old Tank ,
repair/reconstruction of civil foundations and construction, testing, installation, commissioning and
calibration of new crude Oil storage tank. .

Scope & Objectives

The objective of this PLAN is to define the Health, Safety and Environment (HSE) obligations
associated the current operation at the KUNAR re-construction crude storage Tank Project and to
implement these HSE obligations by the HESPAK during the compression project. HESPAK obligation
is to ensure that APPLICABLE LAWS relating to HSE are respected, complied with and monitored
during the entire performance. HSE criteria describe the basic requirements to ensure an inherently
safe design of the plant and related facilities. HSE criteria include facilities design, equipment design
and equipment selection in order to:

 The risk and the consequences of accidental events


 Ensure a safe working environment for safe personnel
 Ensure that adequate means for personal protection and escape are provided.
 Provide sufficient safety devices and redundancy to isolate and minimize uncontrolled releases of
flammable and/ or toxic process gases/fluids.
 Provide adequate fire protection systems rapidly detect, control and extinguish any reasonable
foreseeable fire which could develop during normal operation.
 Minimize the potential for pollution of the environment from accidental spills or venting of hazardous
material.

This Plan will be used as a safe instruction to guide the operation and the construction for KUNAR
reconstruction crude storage Tank project. It also sets out measures, based on the HSE management
procedures of HESPAK and local laws regarding HSE as well as owner’s safety regulations, to ensure
through the whole course of contract period protection for personal, safe working condition and
prevent or minimize the losses due to unsafe actions or factors and negligence of activities.

HSE Policy
HESPAK HSE Policy is aimed at identifying and controls safety, Health & Environmental hazards,
accident prevention, occupational health and pollution control and assurance programmed to protect
from injuries and health hazards.HESPAK adheres to the client’s HSE Policy including rules/
regulations and take all necessary protective measures with a view to ensure its effective
implementation safeguarding the security of its employees and all those who directly come in to
contact in any capacity during work.

Promises

 All activities shall be carried out in compliance with local laws regarding HSE and OWNER’S safety
regulations which set out in document and HESPAK promises

 To run effective operation of HSE management system throughout Kunar re-construction crude
storage tank project.

 Design carefully and carry out construction strictly to protect personnel’s health and safety and
natural environment.

 Provide strong leadership and necessary recourses for HSE work.

 Protect social and public interests, establish excellent enterprise culture and make efforts to improve
the HSE performance level of the company.

Objectives

The top objectives for HESPAK HSE management is to seek no accident , pollution, damages and try
to prove ourselves as the first class international company by the satisfactory of our client.

SUB CONTRACORS with all APPLICABLE LAWS relating to HSE and HSE standards mentioned
throughout the performance of Kunar re-construction of crude storage tank project. Industry
standards shall be also respected and in compliance with. HESPAK shall comply and ensure
compliance by any of its.

Safe Rules on Site

Personnel on Site:

Access of personnel to the work sites is controlled to ensure that no personnel commence work on
the site without being registered in the muster list, wearing the proper personal protective
equipment, and receiving a suitable HSE induction safety course.

3.0 Introduction and Objectives

This Health, Safety and Environmental (HSE) Plan sets out the manner in which Hespak intends to deliver the
commitments set out in the Company Health, Safety and Environmental policy statements, the requirements
of accepted industry codes of practice and the requirements of the client and Consultant on the project.

The principles of control for the safe execution of the contract as outlined in this project HSE plan. Further
detail as to how the Hespak will control safe execution on the project can be found in the HSE work
procedures and standards. UK Health and Safety Regulations, whilst not law are adopted as good practice but
the plan is based on the following principal legislation (this is not exhaustive):

 Fire and life Safety code of practice 2017
It is the objective of High Zone to provide a safe working environment for all persons affected by its activities.
Hespak will therefore promote a ZERO HARM culture. Zero Harm Means:

 Zero Fatalities

 Zero injuries to the public

 Zero seriously disabling injuries to workforce

 An Accident Frequency Rate reportable below 0.01 :- whilst working towards AFR target Zero
ZERO HARM focuses on continually improving the management of health, safety and welfare and
environmental issues within the organisation.

3.1 Purpose

This plan is to define the general guidelines established by Hespak, to provide all personnel with safe operating
practices and awareness for the work they perform in the course of their duties during the project.

The purpose of this plan is therefore:

 To explain the rules set by statutory authorities, Client and Hespak to monitor and improve the
performance regarding health and safety of employees at work and environmental protection.
 To ensure the legal and other requirement.

3.2 Scope

This plan applies to “detached retail Units Hespak” site and office.

4.0 Distribution and Maintenance

This HSE Plan shall be distributed to Project Management for approval. Once approved it will be distributed to
key personnel on the project site. Hard copies will be issued to subcontractors.
The Project and Safety Leads shall ensure that the plan is reviewed to ensure:
 The objectives and requirements of the plan are still valid, and are being met.
 forthcoming activities are reviewed and any necessary amendments to the plan are put in place
before the relevant work begins
 HSE processes shall be reviewed to ensure continuing suitability and effectiveness.

6.0 Duties and Responsibilities

Site Manager
The Site Manager is responsible for the overall HSE management of the contracts undertaken by him. He is
responsible for:
 Ensuring knowledge of and compliance with company policies, procedures, Hespak Health and
Safety standards and site safety rules.
 Being visible in the Health and Safety process, constantly monitoring the project environment
and performance.
 Provide the force and resources necessary for project compliance with agreed safety procedures
and standards.
 Taking action to correct discrepancies reported as a result of surveys or inspections.
 Ensuring that all employees (including sub-contractors) are competent to perform their work
tasks.
 Providing safe plant, equipment and working conditions for all employees.
 Appointment of key personnel
 Ensuring that resources allotted for HSE activities are being properly utilised.
 Highlighting safety priorities.
 Monitoring compliance with agreed safety procedures and standards.

Project HSE Engineer

The Project HSE Engineer report to the Site Manager. He is responsible for:

 Ensuring compliance of company HSE Management system.


 Keeping the project management informed of the status of Health, Safety efforts and advising
the project management team as to the requirements.
 For corrective action or measures necessary to carry out implementation of HSE policy to its fullest
extent.
 Verifying the Method statement and Risk Assessment.
 Forcefully executing his function without tolerance for noted complacency or non- compliance of
anybody on the project.
 Ensuring that habitual violators or safety complacent site personnel are reported to senior
management.
 Conducting and participating in project safety meetings.
 Coordinating and conducting the safety training.
 Investigating all accidents, injuries, fires, property damage, near misses and other safety related
incidents, issuing or disseminating required reports and ensuring that recommended
preventive measures are implemented.
 Evaluating the need for and requesting specified safety equipment.
 Issue of Action Notices in cases where violation of safety procedures are noticed.
 Maintain the Project HSE documentation.
 Ensuring knowledge of and compliance with company policies, procedures, Hespak and Safety
standards and site safety rules.
 Advise on and assist with the implementation of corrective action or measures necessary to ensure
site safety.
 Coordinating site first aid and rescue procedures.
 Coordinating and conducting safety training in accordance with the project requirements.

Supervisors must be knowledgeable in regard to all aspects of Health, Safety and Environmental practices on
their projects. Supervisors should know all of the aspects of the work for the work they are responsible and
the control measures which need to be in place prior to and during the work. Supervisors are responsible for:-
 Understanding their personal duties and responsibilities.
 Supporting the project Health, Safety and Environmental process.
 Preventing HSE being circumvented or undermined.
 Instructing personnel in HSE requirements and practices applicable to the work as documented-
tool box talks that they will be performing through instruction and on site guidance. This should
be daily and before the start of any new task and documented
 Workplace and operative liaison on matters of safety.
 Identify safety training needs.
 Ensuring that all required safety equipment is requested and made available to those carrying
out the work on site.
 Reviewing HSE considerations for all job tasks and recognizing potential risks before starting.
 Discussing any areas of concern with the relevant leader/manager/ HSE personnel.
 Ensuring that Risk assessments are carried out by the relevant engineers prior to the execution
of specific tasks.
 Ensuring that method statements incorporate risk assessments are in place prior to the
execution of specific tasks.
 Carry out independent documented HSE inspections minimum once in week at the respective
area/zone/project
 Setting a personal example.

All Personnel / Operatives


 Ensuring knowledge of and compliance with company policies, procedures, Hespak Health and
Safety standards and site safety rules.
 Personnel / operatives must be knowledgeable in regard to all aspects of Health, Safety and
Environmental practices on their projects.
 They must be aware of the nature of work that has unusually high incidents or accidents. They
are responsible for:
 Understanding their personal duties and responsibilities
 Supporting the project Health, Safety and Environmental process.
 Preventing HSE being circumvented or undermined.
 Ensuring that all required safety equipment is requested, used accordingly and protected from
damage or loss.
 Ensure themselves fit for work and report to supervisor/manager prior to starting the works if
any health issues and unfit for work.
 Ensure the task briefed and fully understand the task, associated risks and controls along with
rules and follow the rules.
 Report all unsafe events and conditions and take care of them self and others

8.0 Site Health, Safety and Environmental Control


8.1 Scope of Works
The scope of work of the project comprised Civil and Mechanical activities for Dismantling and re construction
of crude Storage tank .

8.2 Safety Induction


On arrival all operatives and sub-contractors operatives and visitor will report to the Company site office
where they will attend the High Zone site HSE induction. The induction will be delivered by the project HSE
team and include the following as a minimum:
 Civil and Mechanical installation details – type of equipment, site layout, locations etc

8.3 Competence and Training


All Hespak operatives, inclusive of its subcontractors labour, will be competent to carry out their duties.
Safety and specialist training will be provided in house or via company approved external trainers as necessary
to ensure that any specific project requirements are met. It will be the responsibility of the Project
Management Team to ensure that all site operatives engaged in certain operations hold valid certification. The
project HSE team will check competence certification during routine inspection.

Prior to setting any employee to work the Project Management and Supervisory teams should satisfy
themselves where possible, that all persons are physically, medically, and emotionally fit for performing the
duties to which they are assigned. Consideration should also be given to the employee’s strength, endurance,
agility, and coordination, visual and hearing acuity.

8.4 Method Statements and Risk Assessments


Safe Systems of work are necessary for all tasks and will be generated and submitted to Project Management
and consultant for approval. Method Statements and Risks Assessments shall be produced by the relevant

discipline manager/engineer or Supervisor. When produced they will be forwarded to the project manager and
safety lead for review. Once the project team are satisfied with the content they will then be forwarded to
Project management and Consultant for review and approval. Once approved the work sequence, risks and
control measures will be documented.
Work will be stopped immediately should any team be found not to have been briefed on the safe system of
work or be in possession of the task briefing sheet. The findings of the Risk assessment will be communicated
to all employees and their signatures will be obtained on the Training Attendance Register as proof that they
have been made aware, that they understand the identified risks and hazards, and to ensure that they will
comply with the control measures put into place.

8.5 Tool Box Talks


Tool box talks at site shall be conducted as a minimum on a weekly basis with records of these retained for
auditing purposes. Hespak shall ensure all Engineers attend at least one Toolbox Talk per Month. The
minimum accepted supervisory level for conducting Toolbox talks shall be a Supervisor/Foreman/charge-hand
and topics for these talks shall be related to the relevant hazards and risks employees

8.6 Task Daily Briefings


Prior to the commencement of the working shift task briefings will be carried out by the Supervisor, Foreman
or Charge-hand to those under his responsibility. Daily briefings will include any information relating to the
daily task / activity to be carried out and will ensure that those personnel under the charge of the Supervisor,
Foreman or Charge-hand understand the associated risks and hazards. Daily Briefings will be recorded and
submitted to the HSE Engineer each day for compliance and audit. The daily briefing should be the platform at
which the method statement and risk assessment briefing take place when required.

8.7 Drugs and Alcohol


Hespak has zero tolerance towards alcohol and drugs. At no time while on duty may employees use or be
under the influence of alcohol, narcotics, intoxicants, or similar mind-altering substances. Employees
suspected of being under the influence of or consuming such substances will be immediately removed from
the job site and tested accordingly. If the test proves to be positive, disciplinary action will be taken which may
lead to dismissal.

Adequate sanitation facilities, sanitary sewage and water source shall be provided in the workplace in
accordance with the Dubai Municipality Code of construction safety practices.

8.8 Subcontractor Control


All Sub-Contractors working on Project shall have in fact a HSE plan responsive to the project’s HSE
requirements and shall designate a HSE Representative.
Sub-contractors will be required to provide Hespak with appropriate Method statements and / or Risk
Assessments for all elements of work prior to commencement. Copies of the relevant competence and training
certificates will be requested prior to the employee being put to work. All Sub-contract personnel will attend
the Site HSE induction.
Sub-contractors will be subject to routine inspection by Hespak's HSE Team at any time. Repeated failure to
comply with site and Hespak's health and safety rules will result in removal from site.

8.9 Protection of the Public and Other Trades
Hespak will ensure that visitors and members of the public are protected from injury when visiting or in close
proximity to the site. Hespak employees and Sub-contractors will not disturb, remove or misuse any barriers,
guards, warning signs, walkways, hoarding, fencing, lighting, netting etc that have been erected on the site
project.

8.11 Plant Vehicle Movement


Wherever possible Hespak will ensure all employees will never be less than 3m away from moving mobile
equipment.
The Site Manager will appoint a person to manage Plant, Vehicle, Pedestrian movements at the areas for
which Hespak has control. This will include a traffic management plan and the placement of physical barriers
to segregate people and plant.
Plant Vehicle Marshalls will be trained and appointed as required. Plant and Vehicle movements must be
planned and appropriate plans displayed and brought attention to all through induction, trainings and
meetings.

8.12 Work at Height


Hespak will never work at any height where there is a risk of injury from falling or in any situation 2m (6ft) or
more above a surface unless fall prevention measures are in place.
All work activities where there is a risk of falling a distance liable to cause personal injury must be considered
as working at height. This includes access to and from the place of work, loading/unloading deliveries, working
in service/lift risers, roof work, excavations, etc. This is regardless of the; work equipment being used; the
duration the person is at height; the height at which the work is performed.
Project Leads will ensure that any work being carried out at height is:
 Properly planned, including the selection of work equipment
 Appropriately supervised
 Carried out in a safe manner

8.13 Safety Harness Requirements


Safety harness will be used at all heights above 1.8m where there is a danger of falling. For heights above 6m a
fall arrest lanyard will be used, for heights below 6m a restraint lanyard will be used.
Operatives required to wear a safety harness should be trained in its use. Visual daily inspections should be
undertaken by the wearer.

8.14 Ladders/Stepladders
Ladders and Stepladders may only be used once proven that it is not reasonably practicable for any other form
of access to be used. They may be used for work of a short duration (minutes) and only when in receipt of a
permit issued by the project safety team. Stepladders (under permit) will be footed by a second operative if
working above 2m.

8.15 Scaffolds
Scaffolds and mobile towers can only be used when an in date and signed scafftag is in place which indicates
the scaffold is fit for purpose. Scaffolds must only be used once erected and inspected by a competent person.
Alterations to scaffolds of any type are prohibited unless undertaken by a competent person. A documented
inspection must then be undertaken prior to use.
8.16 MEWPs
Only trained and competent persons will be permitted to use a Mobile Elevated Work Platform. All occupants
of the MEWP will be required to wear a fall restraint lanyard. Where space restrictions allow all stationary
MEWPS will have a barricaded exclusion zone at the front and rear to prevent accidental contact with
pedestrians.

8.17 Excavations
Hespak will never work in or around unsupported ground of any depth where there is a risk of injury from
ground movement or in any situation 1.2m or more below the ground surface unless ground support measures
are in place.
Hespak will ensure suitable barriers will remain in place around the excavation at all times along with
adequate warning signage. Hespak recognises that barriers act as edge protection and an exclusion zone and
will therefore ensure barriers are at least 1 meter back from the excavation edge.
8.18 Confined Space
All work requiring entry into confined spaces will only be undertaken by trained and competent persons.
Access to confined spaces can only be gained once in receipt of a permit issued by an authorised person once
the following requirements have been met:
 Permit is communicated to all affected personnel and posted, as required.
 All people involved are competent to do the work and must be trained first.
 All people involved are equipped with the appropriate PPE and safety equipment.
 Testing of atmosphere is conducted, verified and repeated as often as defined by the
risk assessment.
 Stand-by person is stationed.
 A rescue plan is in place.

8.19 Lifting Operations


Hespak will never lift a load that exceeds the capacity of the equipment or perform a lifting operation when a
person is in the drop zone.
Hespak will follow the lifting procedures at all times. As a minimum requirement all lifts using mobile or tower
cranes require a lift plan and method statement. As High Zone do not specialise in lifting techniques using of
mobile or tower cranes all lifts will be contract lifts. Subcontractors will be responsible for the production of
the lift plans which will be submitted to Hespak appointed person for approval prior to the lift being
authorised.

All routine lifting operations will be subject to the production of a method statement by the Site Manager
appointed person where the lift involves any of the following equipment:
 Chain block
 Web sling
 Vertical material lift (e.g. genie lift)
 Use of telehandler / forklift as a lifting device
 Hiab/lorry mounted crane
A qualified slinger / signaller will be available for all loads lifted with a crane to ensure all loads are slung
correctly. Lifting gear and accessories will be certified by a third party; approved by the Pakistan Government.
Certificates will be retained in the Hespak with the HSE Engineer.

8.20 Plant, Tools and Equipment


Plant, tools and equipment will only be issued to competent operatives trained in its use and inspection. All
equipment will be signed for by the operative and a record of use maintained. Portable appliances, including
leads, supplied by Hespak or its Subcontractors will be inspected prior to use and on a quarterly basis by a
competent person. Daily visual inspections will be undertaken by the user. Damaged or faulty equipment will
not be issued or will be returned to site stores immediately and quarantined. All inspected tools will be colour
coded to reflect the period of inspection.
Plant and Equipment which is powered by a combustion engine will follow the criteria below:
Refuelling
Vehicle to vehicle fuelling is only permitted when one of the vehicles is a fuel bowser. Refuelling activities can
only be undertaken following the process below:
 Fuel canisters must be marked.
 Fuel canisters must be protected from sources of heat and stored out of direct sunlight.
 Use appropriate flexible or rigid pouring spout as required
 Fuel tanks will not be filled whilst engines are running.
 Must not open or decanted within 10m of a naked flame.
 When re-fuelling or decanting, maintain a safe distance of at least 3m away from other workers.
Placement of plant with combustion engines
All static plant with combustion engines must follow the guidelines below:
 Must be placed on drip trays
 Spill kits will be available to absorb leaking of fuel or lubricant
 Exhaust pipes will be kept clear of combustible materials and where practicable be separated from
working areas and other buildings.
 Where ever possible, Plant and Equipment will be protected from accidental impact.
 Air intakes will be situated so that the air is cool, uncontaminated and free from flammable gases and
vapours.

8.21 Control of Substances Hazardous to Health (COSHH)
The use of substances hazardous to health will be considered in the task specific risk assessment which will
detail the control measures to be implemented. All work requiring the use of hazardous substances will have
an MSDS sheet attached to the Work Activity Plan. All operatives carrying out work using the substances will
have been briefed on the detail contained in the MSDS. If necessary a COSHH Assessment will be developed by
the project safety lead.
Designated COSHH Store will be used to store hazardous materials to prevent risk to health and fire. Materials
will be stored, handled and disposed of in accordance with the guidelines on the MSDS and COSHH risk and
Impact Assessments. Care will be taken to order and store small quantities to ensure the risk posed by
hazardous substances is as low as reasonably practicable.
8.22 LPG / Compressed Gas Cylinders
All cylinders will conform to International standards, will be maintained in good condition and will only be
provided with protective guards or caps for the valve assemblies. Compressed gas cylinders will only be
operated and handled personnel who have been instructed in the procedures for their use and the hazards
associated with them.
They will be stored and transported in the upright position and be placed in sectional boxes or containers in
such a manner that they cannot knock against other cylinders or obstructions. Storage areas will be shaded,
well ventilated and made of non-combustible material. Storage areas must remain locked at all times and
display relevant signage and appropriate hazard symbols. Suitable fire fighting equipment, extinguishers,
hydrant etc will be stored directly in the vicinity of the cylinder storage area.

8.23 Noise
Where possible, noise levels will be kept to a minimum at all times. Hearing protection will be provided to all
operatives if requested and be mandated for all tasks where risk assessments identify that noise levels may
reach in excess of 85dBA. Periodic noise assessments will be carried out by the project safety team and if
necessary noise exclusion zones will be implemented.

8.24 Heat Stress


The Project Lead will ensure that Hespak will adhere to the Pakistan regulations for summer working hours. He
will ensure that all operatives have been briefed on the requirement and control measures for working in
extreme heat conditions. All operatives will have access to sufficient quantities of electrolyte drinks and have
sufficient rest periods.

8.25 Electricity
Hespak will never work on a live system unless the energy source has been isolated and verified through
testing.
Plant/switch rooms, transformer banks and high voltage equipment shall be protected from unauthorized
access. Entrances not under constant observation shall be kept locked. Gates or doors to enclosures for
electrical equipment shall swing outward or provide clearance from installed equipment.
All power tools and electrically operated plant will be double insulated. Transformer banks or high voltage
equipment shall be protected from unauthorized access. Entrances not under constant observation shall be
kept locked.

Hespak will appoint a competent electrician for the maintenance and inspection of all Company electrical
equipment / services. Inspections will be documented and copies held by the site Safety Officer.
8.25 Pressure Testing / Pressurised Systems
Hespak will never work on a pressurised system unless the energy source has been discharged and verified
through testing.
Wherever Hespak Personnel, its subcontractors or third party inspectors are required to work on, or with,
pressurized systems, pipe work or equipment that contains either liquid or gas (including air) under pressure,
Hespak will ensure the appropriate control measure are in place.
The Site Manager will designate an authorized competent person assess and supervise all works on pressure
testing/pressurised systems.

8.27 Hot works


All hot work activities will be carried out in accordance with the DM Code of safety practices. Hot works as a
minimum must have a method statement, risk assessment and permit that detail the control measures to be in
place prior to the work commencing.
All operatives involved in hot works will hold the relevant competency certification and will wear PPE as
detailed in the risk assessment.
Where processes include work that generates heat or sparks, where possible it will be done so at a safe
location. Where not possible the works location will be made safe by ensuring all combustible materials and
persons who may be affected by the work are removed from the location or protected from the effects of the
work. All hot works equipment will be inspected daily and defective equipment will be removed from service
and replaced. A fire extinguisher and a trained fire watcher must be present at all work locations.

8.28 Work shop on site


A suitable and designated area shall be identified prior to setup workshop inside the site where necessary, as
to support day to day any MEP service/maintenance /logistics provisions. Fire Safety risk assessment shall be
carried out prior to setup workshop on site.

8.30 Audit and Inspection


Internal Audits shall be conducted on Hespak HSE management systems and compliance. Similarly Hespak is
responsible to conduct audits on all Hespak Sub-Contractors with the audit results documented and filed for
audit purposes.
Actions arising from inspections will be discussed at the meetings and will only be closed out by the project
safety lead once evidence is provided.
8.31 Accident / Incident Reporting
All incidents, accidents, near misses and hazards will be reported to the project lead in order to:
 Establish a written record of factors which may have led to the incident or near miss.
 Investigate incidents or near misses in order to implement corrective/preventative action.
 Provide statistical information for use in analysing all incidents involving Hespak personnel and its
sub-contractors to monitor performance against objectives.

All accidents and incidents shall be immediately reported to DMCC.

8.32 Accident / Incident Investigation


Accident / incident investigation will be conducted. All lost time accidents will be investigated by the project
safety lead. Major and fatal accidents will require senior personnel including Site Manager to perform the
investigation. Recommendations for corrective and/or preventative actions necessary to reduce or eliminate
hazardous conditions will be generated. Investigations will be recorded and documented. Findings and
recommendation will be formally reported.

8.33 Communication and Consultation


Hespak will ensure that all employees are provided with adequate information with regard to health, safety
and welfare on commencement and during their employment on the project. Hespak use various ways of
cascading information including but not limited to: safety inductions, stand downs, daily briefings and toolbox
talks. Project notice boards will display relevant safety information, safety alerts and accident statistics.
Hespak employees have the right to bring health, safety environmental and welfare concerns directly to the
attention of the Project Manager, HSE Engineer or their appointed deputies, who will be obliged to investigate
all complaints or concerns brought forward by any employee.

Hespak's will not allow any of its employees to work alone unless there are effective arrangements in place to
locate and rescue any person that may become incapacitated. Hespak’s work practices generally require a
minimum of two persons undertaking any task. On the rare occasions that lone working may be required it will
only be done following risk assessment. A safe system of work will then be undertaken which will ensure
regular communication and effective arrangements are in place to rescue them should they become
incapacitated.

8.35 Late/night hour’s works


The hazards identified during the Late/night hour’s works and the required control measures are detailed out
in the Risk Assessment for each activity.
Activities are being carried out in the darkness and all safety documents such as Method Statement, and Risk
Assessment prepared for night activities shall be read in conjunction with Method Statement for the activity
while working in the darkness (Late/night hour’s works).
All points of exit, pathways and Assembly points shall be clearly illuminated. The illumination shall be
appropriate for the nature of the job and prior to the work. Access and egress shall be clearly illuminated prior
to start any Late/night hour’s activities.
The area shall be illuminated which will avoid the blind spots and shadows. Are all lighting and light at work
shall be appropriate as per the standard. Illumination at work shall be suitable and sufficient which should not
cause any harm or eye strain.
No one shall be allowed to work in without proper illumination or improperly illuminated area.
No Late/night hour’s works shall be permitted in night until proper safety, security, first aid and supervision
arrangement in place.
Maintenance team for the temporary electrical systems shall be assigned at site during Late/night hour’s
works

8.36 Personal Protective Equipment


Personal Protective and Life Saving Equipment
The management shall provide at own expense, the protective clothing including coverall and all other
personal protective equipment required for different operations. The management shall take necessary
measures to ensure that protective clothing and other personal protective equipment are effectively worn.
PPEs to checked on regular basis and changed when defected or damaged.

8.36.1 Head Protection


Employees working in areas where there is possible danger of head injury from impact or from falling or flying
objects or from electrical shock and burns shall be protected by protective helmets.
Helmets for the protection of employees against impact and penetration of falling and flying objects shall meet
the specifications contained in Guidelines.

8.36.2 Hearing Protection


Whenever it is not feasible to reduce the noise levels or duration of exposures to those specified, ear
protection devices shall be provided and used as detailed in Technical Guideline.
Ear protection devices inserted in the ear or earmuff shall conform to International Standards and shall be
fitted individually.

Plain cotton is not an acceptable protective device.

8.36.3 Eye and Face Protection


Employees shall be provided with eye and face protection equipment when machines or operations present
potential eye or face injury from physical, chemical or radiation agents.
Eye and face protection equipment required by this section shall meet Technical Guideline.
Employees whose vision requires the use of correction lenses as in spectacles, when required by this rule to
wear eye protection, shall be protected by goggles or spectacles of one of the following types:
Spectacles whose protective lenses provide optical correction.
Goggle that can be worn over corrective spectacles without disturbing the adjustment of the spectacles.
Goggles that incorporate corrective lenses mounted behind the protective lenses.
Face and eye protection equipment shall be clean and in good repair. The use of this type of equipment with
structural or optical defects shall be prohibited.

8.36.4 Protective Clothing


Every contractor / employer shall provide adequate and suitable protective clothing for any person so
employed from hazards such as chemical liquids and vapours, excessive heat and cold, electricity, impacts,
ionizing radiation and also general weather conditions. Reference shall be made to Technical Guideline for the
selection and use of protective clothing for various operations.

8.36.5 Hand Protection


Every contractor / employer shall provide adequate and suitable hand protection for any person handling
rough, abrasive, sharp or excessively coarse material such as reinforcement rods, pre cast concrete etc., toxic
and corrosive materials, electricity, and hot materials.
The material of the protective equipment shall be suitable for the operation ensuring proper protection
against the hazard, as detailed in Technical Guideline.

8.36.6 Foot Protection


Contractor / employer shall provide safety-toe footwear to all employees engaged in production and
construction activities, to protect against any possible foot injuries.
The type of safety-toe footwear shall be selected taking to consideration the hazard or combination of
hazards. The requirements and specifications shall conform to Technical Guideline.

8.36.7 Safety Harnesses, Lifelines and Lanyards


Lifelines, safety harnesses and lanyards shall be used only for employee safeguarding. Any life line, safety
harness actually subjected to in-service loading as distinguished from static load testing, shall immediately
remove from service and shall not be used again for employee safe guarding.
Lifelines shall be secured above the point of operation to an anchorage or structural member.
Safety harness lanyard shall be of minimum 1cm thick nylon or equivalent with a maximum length to provide
for a fall of not greater than 2M.

8.36.8 Respiratory Protection

a) General
When controls required either fail or are inadequate to prevent harmful exposure to employees, appropriate
respiratory protective devices shall be provided by the employer / contractor and shall be used.
Respiratory protective devices shall be of the type and standard for the contaminant to which the employee is
exposed.

b) Requirement for a minimal acceptable program


Written standard operating procedures governing the selection and use of respirators shall be established.
Respirators shall be selected on the basis of hazards to which the workers are exposed.
The user shall be instructed and trained in the proper use of respirators and their limitations.
Where practicable, the respirators should be assigned to individual workers for their exclusive use.
There shall be regular inspection and evaluation to determine the continued effectiveness.

c) Respirator Selection
The chemical and physical properties of the contaminant as well as the toxicity and concentration of the
hazardous material shall be considered in selecting the proper respirators.
The nature and extent of the hazard, work requirements, and conditions as well as the limitations and
characteristics of the available respirators, shall also be factors considered in making the proper selection.

9.0 Environmental
The Site Manager will appoint designated store personnel to ensure the following rules apply to materials
stored on site:
 Materials and consumables will be protected from the elements and stored on pallets and covered.
 Items will be stacked safely and secured.
 Flammable substances will only be ordered in small containers and will be stored in shaded location out
of direct sunlight.
 No fuel will be stored in the Hespak stores.
 Heavy items of equipment will not be stacked or stored at high level and will be stored so that easy
access is available for the forklift.
 Hespak stores compound will be maintained in a clean and tidy manner. Records of stores issues will be
maintained.
 All Materials located on site will be stacked in lay down agreed areas.
Storage of combustible materials will be carried out in accordance with the Manufacturer instructions.
Packaging materials will be removed from site at the earliest opportunity in order to reduce the fire load.

9.1 Waste Management and Ground Contamination


Hespak will nominate a housekeeping team specific to the project zones where appropriate. If required by the
terms and conditions of the contract, Hespak will provide the appropriate skips to allow waste to be managed
accordingly.

Waste materials will be removed from site as soon as possible and placed in the designated skip. waste
contractors to remove and dispose of waste from site.
Any hazardous materials/substances for use at the workface will be issued in small containers to limit
environmental damage if the material is spilled or discarded. Contamination of the ground may occur due to
the spillage of oils or the release of chemicals into the ground. No oil, diesel or petrol will be stored on site in
order to eliminate the possibility of unplanned spillage.
During cleaning and flushing works chemicals will be used to clean the pipe work systems. Contaminated water
from these works will not be released into the drainage system. Contaminated water will be stored and
removed from site for disposal at a licensed facility in line with local municipality guidelines.

9.2 Combustible Waste


The requirement to establish and maintain a high standard of good housekeeping throughout all areas of the
Project site work areas will be promoted, initially, during the Project Site Health and Safety Induction.
Combustible waste and materials will be kept to a minimum by:
 Encouraging Suppliers to minimise packaging and waste materials.
 Not allowing waste to accumulate.
 Managers, Foreman, Supervisors and operatives will ensure that work areas are kept clean and
tidy at all times and all waste materials are placed in the appropriate waste skips or bins.
 Unwanted materials are tidied away at the end of each working shift.
Waste material skips will be clearly identified for the type of waste to be deposited in them. Separate metal
bins with close fitting metal lids will be provided for the disposal of flammable waste materials e.g. oily rags,
etc. Waste skips will be removed and emptied on regular basis. The burning of rubbish / waste materials on
site is strictly prohibited.

9.3 Hazardous waste


Waste materials and packaging shall not be permitted to build up on site. Contaminated waste will be treated
and disposed of through an approved municipality refuse site. COSHH and hazardous substances will not be
disposed of through public sewers or waterways. Only registered vehicles and waste sites are to be used for
the disposal of harmful substances. Permits and documentation will be kept. Housekeeping practices shall be
of a high standard to reduce the risk of fire.

9.4 Cement / Concrete


The mixing of concrete or cement will not be permitted directly on the ground. Mixing areas will be kept clean
and tidy at all times. Open / unused cement bags will be stored appropriately and be protected from
moisture. Open / used bags will be stored in weatherproof containers to prevent windblown cement dust.
Cleaning of equipment and flushing of mixers shall only be done at a designated area. Washing of excess
materials into the ground is not permitted.

9.5 Indoor Air Quality Management (IAQ)


Ideally, the quality of air intake matters a lot, so the air quality management at all place of work are and will
always be taken very seriously. Naturally, we have air flowing in everywhere but the particles and what
contains in the air really matters a whole lot. We identify that this are some of the things that disrupt the
indoor air quality of place:

Breaking of bricks
Volatile chemicals released or opened to the atmosphere.
Working in an enclosed room / area
Uncovered smelling waste materials
Saw dust
Fumes produced from welding activity

To all this, we are tend to put in place some dynamic measure to alleviate any hazards i.e. air quality.
Anywhere, dust / particles are flowing at an above than normal rate. The area will be wet with water
effectively. This will reduce the particles and make the air in the area absorbable. Discretionally, this action
normally will be taken by the workers in that area with the approval of the present supervisor. The reason for
this approval is to make sure water is not applied to some area where it need not to, and again for proper
instruction on the capacity of water to use, because if the water is too much, the area become marshy or
slippery, and any of the two will have consequences.
Covered waste bins will be provided so as to prevent any foul odor that comes out of it. Normally, not all
wastes are smelly waste product, but for example, the remains of foods, chemicals buckets, etc. could have an
impact on the air quality so it’s best not to be opened.
Working in an enclosed area really needs more attention and care, because the IAQ in the area will really
determine the air intake of the workers present.
In an enclosed area, the air instead of circulating, rotate thereby not going thru to bring in fresh air exchange.
However, any enclosed area will be provided with an exhaust fan which will make for the lack of air circulation.
The exhaust fan will however not be facing the person’s working and prevent any particle inhaling.

9.6 Fuels Petrol / Diesel / Oil

Where possible fuels will not be stored on project sites. Where storage of fuels is necessary storage tanks will
be sealed to prevent unauthorized use. Required fuels, oils and petroleum based lubricants will only be stored
in minor quantities to reduce fire risk. Containers in excess of 1 gallon will not be stored outside of a shaded,
bounded fuel store. Fuel and oil spills will be contained and cleaned immediately. Contaminated soil / sand
will be bagged and removed from site for appropriate disposal. The fuel store will be restricted to authorized

displayed. Appropriate number of fire extinguishers / fire protection will be placed around the fuel storage
area and signage displayed

9.7 Plant / Equipment


Plant powered by combustion engines, such as compressors or generators, will be positioned in the open air or
in a well-ventilated non-combustible enclosures and will be protected from accidental impact. Drip trays or
bunds will be placed under all plant which will be used to contain leaking of fuel or lubricant. Spills, drips and
leaks will cleared away immediately and plant / equipment repaired to prevent further contamination or risk.
Fuel tanks will not be filled whilst engines are running.

A detailed fire risk assessment will be carried out after mobilization, all control measures shall be applied and
the risks shall be communicated to all personnel at site & office. Adequate number of fire wardens shall be
arranged cluster wise as per the site requirements.

10.1 Fire Prevention


The following practices should be followed by all employees & visitors as appended below:
 All offices and buildings shall be kept free from flammable materials & debris. All rubbish/garbage shall
be placed in bins/skips away from the offices
 Flammable liquids & gases shall not be stored in offices or site except those specifically constructed for
the purpose
 Fire exits shall be kept clear at all times with escape routes clearly identified
 Regular checks shall be made for fire prevention measures
 Smoking or naked flames are not to be permitted near to combustible/flammable/explosives
 Designated area shall be provided for smoking
 Signs shall be posted for indication of areas

10.2 Fire Protection

The practices mentioned below shall be followed for fire protection


1. All offices & site buildings shall be equipped with fire extinguishers which are marked on the type &
limit of use
2. Any hot works carried out on site shall be assessed for risk and measures like fire extinguishers, sand or
fire blankets shall be made available in advance
3. A fire watch shall be assigned for each hot works
4. Hot work permit system shall be followed
5. Fire fighting training shall be imparted to the work force & the staff
6. Trained Fire wardens shall be deployed adequately
7. Regular checks shall be carried out on all fire fighting equipment
8. ‘No Smoking’ signs will be placed in all office rooms, stores and smoking will be strictly prohibited in
these areas
9. Smoking shall be permitted only in designated areas
10.3 Fire Fighting Facilities

The following fire fighting facilities shall be made available at site & offices
 Fire points comprising CO2 fire extinguishers each in locations 15 M apart in the offices and the site
 fire extinguishers shall be provided in addition to the above near to all generators & flammable liquid
storage area

Fire & emergency assembly points shall be made available, emergency contact numbers shall be posted in
conspicuous places.

This procedure is for managing an emergency situation during the site operations. Developing the plan is the
process of assigning emergency related tasks to the individuals in the project, and outlining the protective
actions to be taken. This plan is consistent with the local government’s emergency operations plan, and is
prepared to provide preparedness and necessary responses to emergencies concerned with the project. The
potential emergency cases in the civil works are:

a. Fire or Explosion
b. Hazardous Material / Chemical Leakage.
c. Heavy Machinery and Equipment Failure Or Collapse
d. Accident/ Incident
e. Personnel injury;
f. Extreme Weather Conditions
g. Facility Blackout.
h. Confined space
i. Electric Shock
This plan describes general issues for emergency preparedness, post emergency, evacuation drills and
emergency prevention.

11.1 Definitions & Abbreviations

 Access Control Point - A staffed or barricaded point established to prohibit access to a hazardous or
potentially hazardous area

 Assembly Area - A designated area, located away from the emergency scene, where persons gather
during an emergency evacuation for the purpose of receiving emergency briefings and instructions,
and to be accounted for. An assembly area may or may not be offsite; the location generally depends
upon the size and type of project Manager involved and the type of surrounding land uses at each
project.

 Disaster - An occurrence or imminent threat of widespread or severe damage, injury, or loss of life or
property resulting from a natural or man-made cause, including but not limited to fire, flood, snow,
ice, tornado, windstorm, wave action, oil spill, water contamination, utility failure, hazardous
peacetime radiological incident, major transportation accident, hazardous material incident, epidemic,
air contamination, blight, drought, infestation, explosion, or hostile military action, or paramilitary
action, or similar effects resulting from terrorist activities, riots or civil disorders.

 Escape Routes: Emergency escape routes will be indicated at locations.

 Emergency Coordinator - Emergency coordinator is responsible for maintaining the project


emergency plan and ensuring that all members are informed and knowledgeable in performing their
tasks before an emergency occurs. During an actual emergency or disaster, the Site Manager
Coordinator does not necessarily make the decisions concerning protective actions, but coordinates
the implementation of all necessary tasks in the project Manager emergency team and make
recommendations to upper management, capable of taking action during an event.

 Emergency Plan - The plan developed and maintained for the purpose of organizing and coordinating
its emergency response activities and operations. The plan is consistent with the Emergency
Operations Plan.

 Emergency Situation - Any situation confronting a project, facility, or community requiring emergency
actions of a lesser nature than a disaster to include, but not be limited to, civil disturbances, labor
strikes, and build-up activities prior to an actual disaster.

 Evacuation - A protective action strategy that provides for the orderly movement of people away from
an actual or potential hazard.

 Fire Warden – Trained Fire warden will be assigned.

 Protective Actions - Those emergency measures taken to protect people from the effects of a hazard.
These may include evacuation and in-place sheltering.

 State of Disaster - A declaration by executive order or proclamation by the Government activates the
disaster response and recovery aspects of state and local emergency plans and authorizes deployment
and use of any forces to which the plan or plans apply.

 Unit Emergency Coordinator - The person designated from each unit to carry out protective measures
(evacuation or protective shelter) at the project.

11.2 Emergency Response Plan

Team members will be involved in both planning and response activities. Teamwork insures all necessary issues
and increases staff awareness of the plan and combines different tasks incorporated in team positions.
Emergency response team for initial response will manage emergency situation and team members will be
equipped with the skill to fulfil their roles. The team will comprise of:

 Emergency coordinator
 First aider
 Site Manager and safety personnel
 Specialist operators

11.2.1 Equipment’s

Survey the project for equipment resources that may be needed to respond to the hazards identified in hazard
analysis. For example, if the project lacks an alarm system or public warning system, what can be done to warn
employees during any emergency? The following safety items shall be provided in sufficient quantities and
maintained properly to ensure working condition when in demand:

 Fire Extinguishers
 First aid apparatus
 Public warning system (Alarm, Bell, Horn, Flashing Lights)
 Mobile Phone or other communication system
 Flashlights spare batteries
 Spill control facilities
 Emergency light and power facilities

The project emergency team will identify what resources local response agencies have available and how those
agencies plan to respond to an incident at the project. In order to respond effectively, the team will work with
the local emergency management coordinator, fire, police, and emergency medical services.

11.2.2 Local Emergency Response Centre

11.3 Implementation Of Emergency Plan

To implement the plan, make the plan known, employees must be trained and the plan must be exercised.
Exercising and testing the Project emergency plan also allows the project emergency team to determine, first
hand, whether the plan will actually work. If certain procedures do not work, then the plan will be revised.

11.3.1 Training

A training program will be developed for all staff and workers.

a. All new employees will be trained during site safety induction.


b. Presentation and discussions will be held during toolbox meetings.
c. Project emergency team will be introduced.
d. How they will react during an actual emergency.
e. Drills - communication, evacuation, sheltering, warning, etc.
f. An example would be a fire drill
g. Warning signal and evacuation tasks will be tested.

11.3.2 What Every Employee Needs To Know?

There are certain pieces of information that every employee needs to know in advance of an emergency.

 Lookout should stay outside of an emergency affected area.


 How to safely evacuate the building using the nearest available exit- Do not use lifts.
 Where fire alarm pulls / Muster points are located.
 Where fire extinguishers are located.
 How the team will be notified.
 Where they should report during an evacuation.
 How they will be notified if they should not report to work.
 How to contact the emergency services.
 Do not re-enter until told it safe to do so

11.3.3 Inspection by competent person

Fire Marshals/HSE Leads / Project emergency coordinator will inspect for all support works and maintain
record of inspections.

11.3.4 Training and Monitoring

Training for emergency response will be provided to all during safety induction. As a special training, an
evacuation drill should be effectively utilized. Continuous monitoring will be performed through evacuation
drills and implementing Emergency Plan. A meeting shall be initiated by the project senior management and
reviewed the emergency drill findings and any areas for improvement. Drill assessment and follow-up actions
to be documented and distributed to responsible persons.

11.4 Fire or Explosion Procedure


Fire and explosion can be caused by accidental circumstances, such as inadequate or damaged electrical wiring,
or poor management and inadequate control measures over the flammable substances. The cause of fire also
can be intentional, as in the case of arson or sabotage. Fires are most devastating when they occur to project
with large concentrations of people, such as office buildings. Heat, smoke, and flying or falling debris may
cause injury or loss of life. Structural damage or collapse may occur if a fire or explosion is strong enough. Panic
may result in further injuries if people attempt to evacuate in a disorganized manner. Vital records may be
damaged or destroyed by the fire resulting in significant economic and legal complications. Essential
operations may be disrupted for a long period of time as facilities are repaired or reconstructed.

11.4.1 Notification and Warning of a Fire and Fire wardens

 Notification of a fire or explosion in the project will be made by fire alarm/Air horn.
 If alarm is not available in the vicinity he should shout Fire... Fire … Fire to inform all people. The
warning signal consists of ringing bell.
 The first person spotting a fire should activate the alarm system/Air Horn
 Telephone call will be made to project HSE/project manager/ emergency services.
 Disconnects electrical power source from any live equipment.
 Guide and support employees/subcontract/occupants/visitors to the nearest exit routes/assembly
points.
 At each assembly point takes head count and report to emergency coordinator.

11.4.2 Emergency Coordinator Checklist

 Notify fire department and determine the extent of the fire or explosion.
 Determine what staff and personnel should do during interim period.
 Authorize the use of organization resources by Project Emergency Team.
 Ensure that the fire alarm system has been activated.
 Activate the Project Emergency Plan.
 Coordinate with maintenance personnel, utility companies, and the fire department in shutting down
utility lines or systems that might present an additional hazard (i.e., Electric, gas main).

11.5 Hazardous Material Incident Procedure


Project located near the public traffic and in areas with other facilities, should be aware of the possibility of a
hazardous material incident and have provisions in place for protecting people at the project. Depending upon
the size and location of the incident and the direction of the plume, either evacuation or in-place sheltering
could be used. The incident could be a transportation accident, explosion, fire, or release of toxic materials
causing environmental contamination. These incidents could cause injury or loss of life to persons coming in
contact with or inhaling the material and adverse impact on the environmental element. Some hazardous
materials cannot be detected by human senses because they are colorless and odorless. These materials are
particularly dangerous because they can cause widespread injury or death without adequate warning and
protective measures.

Work areas using, storing, transporting, or producing hazardous materials may have certain requirements to
follow during a release or spill. These project Managers may choose to add those specific procedures for
project hazardous material incident to this plan.

11.5.1 Notifications and Warning

 Project employees will be warned by air horn siren or whistle.

11.5.2 Emergency Coordinator Checklist

 Authorize the use of organization resources by Project Emergency Team.


 Activate the Project Emergency Plan.
 Ensure that warning system is activated.
 Ensure that all doors and windows are closed, the ventilation system is turned off, and all personnel
remain indoors until it is safe to leave.
 If necessary, evacuate to the offsite evacuation assembly area.
 Dispatch personnel to inspect the project for damage.
 Ensure that non-duplicated records located in the unit are safe.
 Coordinate unit shut-down procedures as necessary.
 Report and maintain a written record of all events that occur during the incident, including actions
taken, decisions made and by whom, personnel involved, costs incurred, etc.
 Report verified damage information to the management.
 Ensure that emergency medical care is provided to injured personnel.

11.6 Utility Outage Emergency Procedure


The loss of one or more major utility system at the project could cause significant operational difficulties and
result in a reduction, or disruption, of essential services as well as economic loss in some cases. The loss of
electricity or the breakdown of plant may cause the scenario briefed above. Specific emergency situation
should review by the emergency coordinator as per the work program and procedure shall develop
accordingly.

11.6.1 Notification and Warning

 The Emergency Coordinator will determine the extent of the utility emergency and contact the
appropriate repair and emergency response personnel.
 Situations of an isolated nature (i.e., power outage or water main break affecting one building or unit)
that can be resolved quickly and safely may only require a partial evacuation or temporary shutdown
of operations.

11.6.2 Temporary Electric Power Failure

 Determine the extent of the power outage.


 Notify personnel to remain at their work areas if it is a short term problem.
 Activate Project Emergency Plan
 Coordinate with maintenance personnel in activating the emergency generator or other alternate
power source.
 Augment personnel and resources as necessary.
 Coordinate start-up procedures with maintenance and electric company.

12.0 Safety Awards and Discipline


The Hespak HSE team will keep a constant check on the methods used by supervisors and employees to
prevent violations. They have the authority to correct and instruct employees concerning the violation of HSE
rules and will stop work in situations of imminent danger. If the HSE Engineer witnesses an unsafe act or work
situation, they will at first establish if the unsafe act has been performed deliberately with a wilful disregard
for the safety of themselves or others. If found to be due to lack of knowledge then the operative will be
briefed on the safe system of work and the supervisor informed. If found to be a deliberate violation the
company disciplinary procedure will be followed, this may lead to instant dismissal.

12.1 Project Site Employee of the Month Award


To encourage and communicate best working practices, achievements, innovative ideas and safe working
practice improvements, the project sites will operate a Project Site Employee of the Month Award. A suitable
award at the discretion of the Project Lead and a HSE certificate of recognition will be presented at the end of
each month to the nominated employee/s by the project and HSE lead.
The Site Manager and Project Site HSE Engineer will be responsible for considering nominations from a
number of sources, including, but not limited to:
 Personal recommendation from a Manager, HSE Team, Supervisor or charge hand.
 Actively contributing to safe working practices on site.
 Identification and reporting of unsafe acts, conditions, accidents and incidents.
 Contribution to project site safety.
 Identification of innovative and new safe working practices and methodologies.

The HSE Award Certificate will be endorsed by the Project Manager and, HSE Engineer, documented and
distributed to responsible persons.
