    3. Executive Summary of the CLUP;
    4. Sangguniang Bayan Secretary’s certificate of Public Hearing
    conducted;
    5. Minutes of Public Hearing/Consultation;
    6. List of envitees and sector represented;
    7. Copy of issued invitation letter/Notice of Public Hearing
    8. Copy of Attendance Sheet
