GENERAL INSTRUCTIONS

1. All the joining forms are in PDF version and it is editable version. PLEASE DO NOT CONVERT

INTO OTHER FORMATS. There is an instruction details on the next page.

2. How to create the Digital Signature – Take a white sheet paper and sign as per pan card and

below the signature mention your DOJ. Take a photo of it and save it in desktop as image format.

3. Please ensure that all the pages of all the PDF joining forms, bottom of the page has the

Digital Signature.

4. All the joining forms expect PF Form 11, PF form 2 and Mediclaim Nomination should have

Aadhar card name rest all have pan card name.

5. In the Mediclaim nomination form – We are collaborated with ICICI insurance vendor – If your

single then the coverage will be employee itself. If you are married then it will be employee,

Spouse and 2 dependent kids only (Name should be as per Aadhar card)

6. Please tick mark wherever Son/Daughter/Spouse is asked please tick mark or highlight it.

7. Whereever the signature is asked, paste the Digital signature, DO NOT WRITE ANYTHING ON

WITNESS DETAILS.

8. In Form 11 – Page 1 : Undertaking – There are 4 options related to the PF withdraw status please

do tick mark accordingly.

9. In PF form 2 Nomination – PART A : Column 4 - Percentage sharing has 3 options

a) Nominating one person with 100%. OR

b) Nominating 2 persons (Father and Mother) with 50-50-%. OR

c) Nominating 2 persons (Father and Mother) with 60-40%


HOW TO EDIT THE PDF FORM

1. Open the file in PDF i.e Adobe Reader DC, once the file is open on your right side there a

fill and sign option, select it

2. Once you select, the below window pops up, once again select Fill and Sign option

3. The document will change to editable version.


4. The first highlighted, if you click on that the cursor pointer will change to text option it

means writing the text. Small A , means decreasing the font size, Capital A means

increasing the font size adjust your text accordingly. If u click on the box edges, you will

see 4 sided arrow marks, it means that you can drag box and choose the proper

alignment of placing the box like left, right and middle.

5. The second highlighted is TICK MARK, click the cursor on the tick marker the pointer

will change. This will be used where ever the option is there to do tick mark, select this

icon.

6. The last or third highlight is adding signature. Please refer the first page ON HOW

TO CREATE A DIGITAL SIGNATURE.

Once you select the add signature, a pop window will open, there select the IMAGE

option. Select the folder where the image is saved, click on open and apply, the curser

pointer will change to signature.
