I, DANNY C. CORDITA , Filipino, of legal age, married, and a resident of

Brgy.Mabua Tandag , Surigao del Sur, state under oath, THAT:

1. I was a holder of Tax Identification Card with TIN number 230-809-556-000

2. I kept said Tax Identification Card in my wallet which contains also other
important documents;

3. However, the wallet which contains the said Tax Identification Card and other
important documents was lost;

4. I exerted all efforts to locate the same but to no avail;

5. I am executing this Affidavit to apprise the BUREAU OF INTERNAL

REVENUE (BIR) the fact of said loss and for whatever legal purpose this may serve
best.

2. I kept said GSIS Card in my wallet which contains also other important
documents;

3. However, the wallet which contains the said GSIS Card and other important
documents was lost;

4. I exerted all efforts to locate the same but to no avail;

5. I am executing this Affidavit to apprise GSIS the fact of said loss and for
whatever legal purpose this may serve best.
