    Self-Assessment of Participation in DH Month Activity

    On a scale of 1 to 10 with 1 being least participation and contribution and the 10 being the most
    participation and contribution rate yourself.

    My rating: ____10_____

    In three to four complete sentences give a description of your participation and contribution to the DH
    month activity,

    As a group, we did Trunk or Treat with kids at Kirkwood. I helped with carrying our supplies to
    where they needed to go and reminding everyone of when our event was. I was in charge of reminding
    everyone of the upcoming date since I was selected as the calendar tracker. I loved being able to
    interact with the kids and talk about their costumes. One of my favorite parts was giving the stuffed
    toothbrush to kids and letting them practice brushing my tooth costume the way they brush their teeth.
