Completing the Payroll Form!

Hey there! As part of your pre-employment requirements, please complete the Payroll Form
(<<click the link) to submit your personal details for payroll processing.

This includes providing the following information:

● Date of Birth
● Previous employer’s email address (for employment verification)
● Government Numbers (SSS, TIN, Philhealth, Pagibig)
● Bank Account Details
○ Bank Name
○ Account Name
○ Account Number

What do you need to know before filling out the payroll form?

● All team members are required to have a BDO personal (savings/payroll) account:

○ If you have a BDO personal savings account, provide your account details by
completing the payroll form.

○ If you have no BDO personal savings account but you have or know someone with a
savings account in any bank for temporary payroll processing:

1. Fill out and sign this Salary Authorization Agreement Form and complete the Payroll
Form.
2. Create a BDO payroll account by requesting an endorsement letter and instructions via
email to the HR Team via HR Helpdesk
○ If you have no personal account or do not know someone with any bank at all,
complete the payroll form and create a BDO payroll account by requesting the BDO
endorsement letter and instructions by sending a ticket via HR Helpdesk
