    Greetings of peace.

    I am writing to express my sincerest apologies regarding the late submission of the Leave Form
    (Form 6) of my teacher constituents in San Jose High School.

    These teachers have submitted their documents on time; however, they have submitted it together
    with their Daily Time Record (DTR). Consequently, it was unintentionally mislook. Nevertheless, I
    have personally talked to the concerned teachers about this matter, and we look forward to preventing
    this from happening again.
