1.   Professional Practice – Safety


Self Mid-Term — Paige Papineau I ensure a patient's safety by always using a gait belt when mobilizing a patient and always asking for help
when I need it. I never get a recent TKA or THA patient up on my own, waiting for my CI to be there. I have
made significant improvements in my ability to adjust my examination and interventions to meet the patient's
current level of functioning. For example, when working with total joints I am able to make predictions of how
well the will do with standing and ambulating based off their level of pain, their muscle control with their HEP,
level of sensation, etc. I am able to complete these safety elements with the majority of the simple patients, but
am requiring supervision more than 25% of the time with complex patients.

Self Final — Paige Papineau Over the final seven weeks, my safety has continued to improve. I ensure patient's safety by using a gait belt,
calling for assistance if deemed necessary, and adapt my plan for the session based on the patient's presentation.
I often use other tasks to determine if a patient is able to progress to more advance tasks. For example, I will
have the patient perform side stepping at the edge of the bed before allowing the patient to ambulate away from
the bed if I am unsure. For TKAs I use teaching of the HEP to help assess when a patient is ready to stand. If a
patient has good strength and control for the duration of the HEP, then there is a pretty good chance that it will
be safe for them to stand and/or walk.

CI Mid-Term — Marie A. Laneuville Paige demonstrates good safety precautions with all her patients, consistently using a gait belt and requesting a
2nd person assist when indicated. She is now able to anticipate and prepare her room set up as she mobilizes her
patients (chair with seat alarm, bedside commode). She will anticipate needs for hip chair for her acute post-
operative patients. Paige has shown improvement managing IV lines and tube drainage. She continues to
monitor for signs of post-operative complications.

CI Final — Marie A. Laneuville Paige has practiced within safety standards consistently throughout her time at Blodgett. She is now able to
determine contraindications to Physical Therapy as indicated by her patients' critical lab values for example.
She requests a second person assist or a chair "spotter" when appropriate. When unsure Paige, always errs on
the side of caution and seeks out help appropriately. She has complied with all physicians' orders for Total Hip
Replacement precautions as well as weight-bearing limitations.

2.   Professional Practice – Professional Behavior


Self Mid-Term — Paige Papineau I arrive on time and am prepared every day. I follow the hospital's dress code, wearing black scrubs every day
and am open to receiving feedback not only from my CI, but other PTs, PTAs, OTs, COTAs, and other hospital
staff. I fill out weekly feedback forms, reflecting on my performance for the week and providing feedback to
my CI. I maintain patient privacy and modesty by following HIPPA, and ensuring the patient is not exposed
through adjusting their gown/blankets, using additional gowns, etc. I am able to maintain professional behaviors
the majority of the time, but am continue to require supervision greater than 25% of the time.

Self Final — Paige Papineau I am always on time, and follow the dress code set by spectrum health. I am always open to feedback from my
CI and other staff. I am open to changing schedules to help others if needed.

CI Mid-Term — Marie A. Laneuville Paige is very punctual and highly reliable, preparing well daily. She has had to flex her schedule multiple times
to accommodate for our fluctuating Total Joint population. Paige is highly receptive of feedback, easily
collaborating with pre and post session discussions.

CI Final — Marie A. Laneuville Paige has displayed outstanding professionalism whether she is interacting with rehabilitation, nursing or care
management staff. Paige has continued to be highly receptive to feedback from both Physical Therapists and
Occupational Therapists (one of greater strength!). As she has become more comfortable, Paige has been able to

provide her patients with a caring, supportive and encouraging approach. Paige always came to work with a
positive and "ready for anything" outlook!

3.   Professional Practice – Accountability


Self Mid-Term — Paige Papineau I am always willing to admit when I am wrong or that something did not go as well as I had hoped because I
want to learn from my mistakes. I always maintain patient confidentiality by abiding HIPAA. An example in
which I place a patient's needs above my self interests and demonstrate generosity was with a very anxious
patient. We were very busy and behind schedule but I took the time to slow down and gain his trust, which
helped make the session smoother and were able to advance the patient further then we most likely would have.
I am improving my accountability each week, but continue to require supervision greater than 25% of the time

4.   Professional Practice – Communication


Self Mid-Term — Paige Papineau I am able to adjust my communication style to meet the needs of the patient. I have used alternative forms of
communication such as utilizing translating services, using white boards/paper to communicate vs. audible
communication. I have worked on communicating with various patients varying in educational levels, cognitive
levels. Additionally I have worked with patients who spoke/understood varying levels of English and with
patients with deficits in vision and hearing. I have communicated with various members of the interdisciplinary
team for effective patient care. I demonstrate active listening though asking clarification questions, verifying
patient information through repeating back what they have explained to me, etc. I recognize that my lack of
confidence can be picked up in my verbal and nonverbal communication and can affect the patient. I need to
improve at initiating communication in difficult situations. I continue to require supervision greater than 25% of
the time with complex patients.

Self Final — Paige Papineau Over the final seven weeks of my clinical, I have made significant improvements in my communication skills. I
have become more confident, assertive, and firm with patient instructions, especially when movement or AD
use requires quick instruction to ensure patient safety. I have gotten better at adjusting my voice for patients
who may have difficulty hearing. Additionally, I have continued to improve my communication with the
interdisciplinary team.

CI Mid-Term — Marie A. Laneuville Paige has been challenged in some aspects of her communication style with our patient population. She has
shown steady progress as she is now able to speak louder and in a more assertive way. She will continue to
direct patient in a firmer manner when safe sequencing requires prompt corrections (ie: "keep your body inside
of your walker at all times", "reach back", "step up with your good leg"). She will continue to "tell patients
what to do" as opposed to asking patients if they "kinda" want to do something. Paige has communicated very
well with her inter-disciplinary team members, clarifying as needed with her CI prior to calling or texting a
nurse, care manager, Occupational Therapist etc. Paige has become more engaged, personable and supportive as
she is more comfortable managing in-patient population! She will continue to emphasize patients' achievement
in order to optimize her outcomes.

CI Final — Marie A. Laneuville Paige has made tremendous improvement in this area. She is now able to communicate in a more self-assured
manner with both patients and inter-disciplinary team members. Paige has been communicating independently
with appropriate staff members after each of her session to ensure proper follow-up care. She has been able to
encourage difficult patients to participate with Physical Therapy providing clear and concise explanation
regarding the benefits of mobility. Paige has been able to adjust her communication to hard of hearing or blind
patients as well as the cognitively impaired. She will continue to provide "firmer" cues to her impulsive or "at
risk" patients. Paige has continued to engage and encourage her patients as well as their family members with
positive communication.

6.   Professional Practice – Professional Development


Self Mid-Term — Paige Papineau I fill out weekly feedback forms evaluating and reflecting on my performance for the week. I have daily and
weekly discussions on my performance, discussing things I do well, things I need to improve on, etc. After
seeing a patient, my CI or other staff provide feedback to help me improve. I write reflection journals every two
weeks reflecting on what I have learned, how I am doing, how my clinical experience is going, etc. By the end
of my clinical I will have to complete two in-services.

Self Final — Paige Papineau I am able to identify my strengths and weaknesses, and seek guidance when needed. I am open to feedback
whether it is from my CI, or another staff member. I have completed weekly feedback and goal forms and have
completed an in-service. I have had opportunities to observe in the IRC. I do need to continue to work on my
confidence.

CI Mid-Term — Marie A. Laneuville Paige is a pleasure to work with! She is highly receptive to feedback, often able to identify her own limitations
while working with more complexed patients, and always requesting assistance or clarification when needed.
She has actively participated in establishing and reviewing our weekly goals. Paige will complete a brief in-
service during her last week of her clinical.

CI Final — Marie A. Laneuville Paige has continued to not only receive feedback extremely well but has recently been seeking feedback
especially when faced with difficult situations (ie. Kristi's). She has been consistent in assessing her strengths
and weaknesses during our weekly "goal meeting". Paige has been an active learner throughout this clinical
frequently asking questions and clarifications as needed. Paige presented an article review to our rehabilitation
staff. Her presentation was well prepared, engaging and professional.

7.   Patient Management – Clinical Reasoning


Self Mid-Term — Paige Papineau I am working to improve my clinical reasoning skills. I am getting better at considering many aspects of the
patient's situation (support at home, current and baseline functional level, home environment, etc) to make
recommendations for d/c planning. I improving my ability to adjust my plan of action/ goals based on patient
presentation. I recognize that I do not know everything and have limited experience, and know when to ask for
help. I am not always confident in my assessment or clinical reasoning when a patient does not meet the
expectation, requiring my CI to assist. I am mostly independent with simple patients, but require supervision
and assist with complex patients.

Self Final — Paige Papineau I have continued to improve my clinical reasoning skills over the last half of my clinical. I am mostly
independent with simple ortho and gen med patients and have gotten better at determining what AD is best for
the patient. While I have made significant improvements in my clinical reasoning complex patients, I do not
think that I am 90-100% independent with this population. However, I am able to maintain the full case load of
what is expected for a new grad.

CI Mid-Term — Marie A. Laneuville Paige is thorough while gathering her patients' prior level of function and is now able to present her rationale
when establishing the Plan of Care for her simple orthopedic patients. She determines their discharge plans as
well as the needs for Occupational Therapy. She continues to require some guidance in determining the best
assistive devices for her patients. She has now improved her clinical reasoning with her simple general-medical
patients and will continue to progress with her more complexed patients as she collaborates more confidently
with her Occupational Therapists.

CI Final — Marie A. Laneuville Paige has continued to improve her clinical reasoning skills. She is able to look at the "whole picture" including
the chart review, her patient interview and her physical assessment to determine her clinical intervention. At
times, Paige will require increased time while working with a complex patient. She is now able however, to
progress her patients to their optimum potential without this CI's assistance. While she continues to appear
underconfident at times, she has consistently established a proper Plan of Care for her patients. Paige will
continue to discuss more complex situations with her inter-disciplinary team.

8.   Patient Management – Screening


Self Mid-Term — Paige Papineau When performing a chart review, I look at the patient's hospital problem, their pmhx, their lab values,
supplemental oxygen needs, precautions, and past notes from members of the interdisciplinary team. I am
improving my ability to determine when a patient may not be appropriate to be seen, but require more
improvement in this area. For patients with recent TKA's, I am improving my ability to screen to determine
whether they have the need to be seen for occupational therapy. I require some cueing for when an outcome
measure such as the tinetti or berg may be indicated for a patient.

Self Final — Paige Papineau Over the last 7 weeks, I have continued to improve my ability to perform a thorough chart review and have
become more independent with screening patients for if they may need to see occupational therapy. I have been
able to execute and properly interpret many outcome measures including Berg, Tinetti, PASS, 6-clicks

CI Mid-Term — Marie A. Laneuville While there are not typical opportunities to "screen" patients in our acute care setting, Paige has been
performing tests such as PASS, Tinetti and Berg to identify patients' fall risk and need for additional therapy.
While she occasionally requires prompting to utilize these tools, she is able to interpret the tests results
independently and choose the appropriate follow-up services.

CI Final — Marie A. Laneuville Paige is now able to determine the need for continued in-patient Physical Therapy as well as the need for
Occupational Therapy consults. When in doubt she has appropriately completed standardized testing including
Tinetti and Berg to support her decision. Paige will continue to discuss complex cases with her inter-
disciplinary team when needed.

9.   Patient Management – Examination


Self Mid-Term — Paige Papineau I am able to complete the subjective portion of the evaluation independently for most patients. From this
information along with information from my chart review, I am able to make predictions on how a patient will
perform throughout the session. For example by learning of the patient's prior level of function, reviewing notes
from nursing and other therapy team members I can have a good idea of how much assist a patient may need.
For example if a patient is typically uses a walker at home and a wheelchair in the community I can predict and
prioritize that the patient will continue to need those assistive devices and that as long as they are able to
ambulate household distances they will be safe at home. Over the past 7 weeks I have greatly improved my
ability to sequence a session. For example when a patient is in bed, I will start with exercises, subjective
questioning vs. if they are up with nursing when I arrive I will start with ambulation, stairs, car transfers then
get them back to bed for exercises and bed mobility.

Self Final — Paige Papineau I have continued to improve my examination skills over the final 7 weeks of my clinical. I am able to obtain a
clear history from a patient, asking clarifying questions appropriately. I have gotten better at chart reviewing,
especially with simple patients, but need to continue to improve my ability to complete a full chart review on
more complex, non-ortho patients as I occasionally only have a vague idea or miss something in the chart
review.

CI Mid-Term — Marie A. Laneuville Paige completes thorough chart reviews but occasionally will require guidance as it relates to prior vs current
procedures (ie. When was the prior TKA? Is a TKA revision vs an Irrigation and Debridement scheduled? For
Lower Extremity Cellulitis, is a Podiatry Consult pending? Did the status-post fall patient undergo a THA vs an
ORIF?). Paige also requires some guidance determining needs for Supplemental Oxygen and monitoring the
Oxygen Saturation rates with her respiratory patients. She is now more consistent describing her patients' gait
and balance in a qualitative manner. Paige thoroughly assesses pain, cognition, range of motion/ goniometry
and manual muscle testing as indicated.

CI Final — Marie A. Laneuville Paige is able to support her rational for mobilizing patients based on her examination. She has continued to
improve her examination skills, especially in the more complex area of respiratory compromised patients as she
is now able to closely monitor her patient's Oxygen saturation and determine the need for supplemental
Oxygen, deep breathing, and pacing accordingly. While this continues to improve, she has at times required
prompting to investigate further into a patient's past medical history for relevant information (ie. prior
admission). Paige will continue to assess her patients gait and balance in a qualitative manner, to "paint the
whole picture".

Self Mid-Term — Paige Papineau Over the past 7 weeks, I have progressed to being able to complete examinations mostly independent with
simple patients, requiring some assist with more complex patients. I have been able to complete subjective
questioning, strength testing, goniometry, sensation testing, assessing balance, gait patterns, need for assistive
device. Additionally, I have improved my ability to recognize when and what vitals I may need to examine
throughout. I have been able to perform evaluations on patients with a variety of diagnoses.

Self Final — Paige Papineau Over that final 7 weeks I have continued to improve in this area. I am independent over 90% of the time with
simple patients, and have significantly improved my evaluation skills for those patients who are more complex.

CI Mid-Term — Marie A. Laneuville Paige is able to gather her assessment information and formulate a Plan of Care for her simple patients. She has
requested guidance when unsure with her more complex patients.

CI Final — Marie A. Laneuville When faced with new complex situations, Paige can be hesitant formulating her clinical decisions. However,
she has been able to consistently formulate proper clinical judgement for both her simple and her complex
patients independently, at times appropriately reviewing her plan with her CI or Occupational Therapist. Paige
will continue to improve her assertiveness as she continues to gain more experience!

11.   Patient Management – Diagnosis and Prognosis


Self Mid-Term — Paige Papineau I have not had to diagnose or consider differential diagnoses much this clinical as the diagnosis is included in
the chart review. However, I have had to hypothesize how a patient's pmhx, co-morbidities, age, health
behaviors may impact interventions and have gained lots of experience with identifying a patient's prognosis,
determining a patient's frequency to be seen, and plans for d/c. I continue to require some support in this area
for more difficult, complex patients.

Self Final — Paige Papineau I have continue to improve my abilities and confidence in this area. I have improved in my ability to predict a
patient's presentation based off of their chart review for both simple and complex patients. I have been able to
properly identify a patient's risk for falls. I have gained lots of experience and opportunity in identifying a
patient's prognosis, and have gotten better and determining a patient's frequency and plans for d/c. I have
become independent over 90% of the time for simple patient's, still requiring reassurance at times from my CI
for complex patients.

CI Mid-Term — Marie A. Laneuville While Paige has not had to determine diagnoses in our in-patient acute care setting, she has been able to
identify post-operative foot drop and determine risk for falls. Paige is able to establish her non-protocol
patients' frequency. She occasionally seeks out CI's input as it relates to the intensity and duration of her
sessions. For example, she may need prompting to ambulate a Day #0 patient in the hallway? Paige has made
great progress integrating all of her patient information in making sound discharge recommendation.

CI Final — Marie A. Laneuville Paige has been able to define her patient's appropriateness for physical therapy and the frequency to achieve her
physical therapy goals.

Self Mid-Term — Paige Papineau As I am in the acute setting, the duration for goals to be met is usually by d/c, but I have been able to set goals
for many patients. I am able to determine when additional goals outside of the normal goals such as goals for
precautions, goals for outcome measures are needed. I have had the opportunity to discuss the risks and benefits
of utilizing different AD for safety and fall prevention. Additionally I have had opportunities to re-assess
patients to determine safety and readiness from a mobility standpoint to d/c to determined destination or if plans
need to be adjusted.

Self Final — Paige Papineau I continue to be consistently independent with plan of care and goal setting for simple patients and have
significantly improved my ability to develop a plan of care for the "unusual" patients who have had joint
replacements. I have gotten better at creating a plan of care and d/c planning for those patients who are more
complex. Will need to continue to improve my confidence in this area with complex patients. Over the last 7
weeks, I have improved my ability to modify a patient's plan of care based on the patient's presentation.

CI Mid-Term — Marie A. Laneuville Paige is able to establish her goals and Plan of Care following her initial Physical Therapy Evaluation for
simple patients. She can now consistently adhere to our Protocol patients' goals, including Total Hip precautions
and Total Knee ROM when indicated. Paige does need some assistance when dealing with patients who are not
presenting with a typical expected outcome (ie. a Total Joint patient who is unable to weight bear at eval due to
severe pain). She is learning to quickly think on her feet and modify her session and her expectations to adapt to
her patients' limitations.

CI Final — Marie A. Laneuville Paige is independent establishing the Plan of Care for her simple and complex population. She will seek out
some guidance/ confirmation when faced with unusual situations such as a Long Covid patient s/p Total Knee
Replacement

13.   Patient Management – Procedural Interventions


Self Mid-Term — Paige Papineau I ensure patient safety with interventions by always using a gait belt and having my CI with me when I feel
unsure or when I need help. I do need to work my efficiency. Interventions that I have performed include HEP,
pt and family education, fitting of AD, stair training, car transfers, bed mobility and transfers, gait. When
performing instruction of HEP, I explain to the patient why the exercises are important. In addition, when teach
stair training, car transfers, etc. I always explain why it is the methods that I am teaching and having them
practice is the most effective and safe way to complete the tasks. I am able to adjust the amount, speed,
presentation of interventions based on the patients learning style, age, co-morbidities, and other factors
impacting patient presentation.

Self Final — Paige Papineau I have become more effective in providing instructions to patients and caregivers, and continue to improve my
ability to change voice/volume to fit patients needs. I have improved my ability to provided prompt instructions
to keep patients safe when they may be impulsive or poor insight of their deficits and need for help. I have
improved my ability to check fit of AD such as walkers and canes and have gotten better at knowing when try
different AD to determine the best AD for the patient.

CI Mid-Term — Marie A. Laneuville Paige has been improving her delivery while instructing her patients and caregivers. She is able to adjust her
voice/ volume and continues to provide prompt cueing when patients are more impulsive or unsafe. She will
continue to "anticipate" and provide those cues in a more expeditive manner. She continues to require some
reminders to fit walkers/ canes to her patients' height, or to try out different walkers in order to recommend the
optimal assistive device. Paige will initiate those interventions over the next few weeks.

CI Final — Marie A. Laneuville Paige is now able to complete and adjust her interventions independently. She has consistently fitted her
patients with the proper assistive devices and may seek out guidance when trialing a new device (hemi-walker,
platform-walker, knee scooter). She is now able to distinguish if a patient should complete a Home Exercise
Program vs advance the patient's functional mobility. She may need occasional guidance when faced with a new
situation such as a transfer for a below-knee amputation patient diagnosed with lower extremity cellulitis.

14.   Patient Management – Educational Interventions


Self Mid-Term — Paige Papineau I have been involved in lots of patient and family education over the past seven weeks. Topics and tasks that I
have educated on include stairs, car transfers, precautions (weight bearing, hip, spinal, brace), safe use of AD,
activity cycle, use of wedge and ice, HEP. I have educated patients through demonstrations, verbally, written
handouts and can easily switch approaches when I identify when I am not being effective with my current
approach

Self Final — Paige Papineau I have continued to educate patients on HEP, activity cycle, safe use of AD, etc. I often change approach of
teaching to what is most effective for the patient and frequently determine if a patient may have OT needs. Not

only have I experienced educating the patient, but also the patient's family/friends/ caregivers on ways to best
assist a patient following d/c. I often use teach-back methods to ensure patient understanding of precautions,
weight-bearing, etc.

CI Mid-Term — Marie A. Laneuville Paige has quickly learned to educate her protocol patients with thorough instructions regarding their
precautions, activity cycle, positioning, icing, Home Exercise Program, and fall prevention. She consistently
provides her patients with a written exercise hand-out when indicated, which she reviews thoroughly. She will
continue to engage her patients and their caregivers using "teach-back" technique and always clarifying if any
additional help is needed.

CI Final — Marie A. Laneuville Paige is incredibly patient and thorough while educating her patients. She has consistently included her patients'
caregivers when appropriate. She has dealt with barriers such as blindness, impaired cognitive status,
emotionally labile or angry patients.

15.   Patient Management – Documentation


Self Mid-Term — Paige Papineau I think documentation is one of my stronger areas. I am able to document accurately ~75% of the time
following the guidelines and format required by the hospital. However, it does take increased time. I have been
involved in documenting evaluations, interventions, plan of care, d/c planning, and communication with others
involved in the delivery of care. Through my documentation, I am able to justify physical therapy services
~75% of the time. I am able to document in an organized manner and am working on improving my efficiency.
When writing the patient's handoff I work hard to ensure they are clear, concise, and provide the most

Self Mid-Term — Paige Papineau Have had the opportunity to complete the Berg, Tinetti, PASS, 6-clicks. I am improving my ability to know
when it is appropriate to administer additional outcome measures outside of the 6-clicks. Additionally, I have
been improving my ability to accurately interpret score of outcome measures.

Self Final — Paige Papineau I have improved my ability to execute many outcome measures (ex. berg, tinetti) as indicated on patients of
varying diagnoses, and have been able to accurately interpret the meaning of outcome measure scores to help
determine are safe to go home, be independent in their rooms, etc.

CI Mid-Term — Marie A. Laneuville Paige has completed "6 clicks" for all her patients, as well as tests including PASS, Tinetti and Berg when

Self Mid-Term — Paige Papineau I have not been very involved in scheduling or billing this clinical, but have gained lots of experience in
coordinating physical therapy with other services such as co-treating with OT, PT eval/treatment and
medication management or patient readiness/ ability to participate with nursing, and treatment/eval with other
scheduled events such as x-rays, MRIs, dialysis, surgery, etc. Additionally, have gained experience with
coordinating need for PT eval/treatment with care managers/social work for d/c planning and insurance
approval. Need to improve on using time effectively. Have improved and need to continue to improve on
prioritizing.

Self Final — Paige Papineau While I am not very involved in billing and scheduling, I have been consistent with recording my in and out
times which my CI utilizes for billing. I continue to improve my ability to communicate with care managers/
social work with recommendations for AD for patients. I am able to accommodate patients' schedules. For
example, when a patient has imaging/testing/surgery scheduled, I try to make it a priority to see them before
their scheduled time.

CI Mid-Term — Marie A. Laneuville Paige has been thoroughly recording her in/out time in order for this CI to complete billing requirements. She is
now more consistent updating our Total Joint schedules and will continue to complete this task independently.
She has reached out to our Care Managers to order patient equipment with some guidance and will continue to

of therapy from PTA or rehab techs. Improving my ability to relay necessary information (and the most
effective and efficient means of doing it) to PTAs, rehab techs, OTs, nursing, care managers, to provide the
patient with the most effective care.

session later during the day. Have utilized rehab tech's when I have needed assistance with needing equipment

Self Mid-Term — Paige Papineau Typical caseload for new-graduates is 19 units or 5-6 patients per day. At this time I am capable of managing
around 70% of a new grad caseload. Sometimes a little less, depending on the day.

Self Final — Paige Papineau Typical caseload for new graduates is 19 units or 5-8 patients/day. At this time I am consistently managing 5-6
patients/day.

CI Mid-Term — Marie A. Laneuville A full caseload for a 9 hour Physical Therapist consists of 5-8 patients/ day for a productivity goal of 19 units/
day. A new graduating therapist would be given a few weeks of orientation to reach this goal. I believe Paige is
on track to meet this caseload as she is currently seeing 4-5 patients/ day for a total productive units of 15 to 17.

CI Final — Marie A. Laneuville A full-time therapist should reach 19 units/ day overseeing 5-8 patients. Paige has consistently met this goal
over the last few weeks.
