Scenario-Based Risk Assessment: Conduct a risk assessment of a workplace
scenario and develop control measures to address identified hazards
Instruction: Read the following scenario and identify potential hazards in the
workplace:
Scenario: You are a safety inspector at a construction site. The site has recently
been expanded and now includes a new building with multiple floors. There are
workers on site who are constructing the building and others who are operating
heavy machinery.
1. Evaluate the risks associated with each hazard identified in step 1,
including the likelihood and severity of harm.

As for mentioned scenario, hazard factors can be identified includes

°Psychological Hazards in which common to a worksite among workers


and sort of man power involves on such project accounted for.

°Equipment Failures, as usual as it is: common practice such as


maintenance of those formention and any sort of equipment should be
the first thing to evaluate before the work started.

°Chemical Hazards, while doing daily checking of those equipment:


potential chemical hazards are also a threats does it is good to be
cautious on checking gasolines or any chemical fluids needed on those
equipments. Included working fluids on those equipment which
considerably a threat such when there is any leaked fluids that are highly
flammable as formention may likely start worst scenario.

°Ventilations Hazards, lets assume that the expansion is quite well


prepared but also let’s not rule out the ventilation hazards that exist
specially when equipments are running as it exhausted fumes and any
sort of equipments that can cause or makes smokes.
°Fire Hazards is quite a threat but less likely to happen assuming that the
design expansion is granted by and well planned. However, it is consider
that working equipments can potentially cause unnecessary fire or
ignition of flammable materials that are used on those equipments.

°Building Load Failure, though we cannot ruled out this hazard or say
that it is unlikely to happen but it is also a threat as the site is newly
expanded and working heavy equipments exist and working.

°Hazards from falling debris is quite common on worksite specially when


heavy equipment exist as failures are thing that cannot be tames most of
the time.

2. Develop control measures to address each hazard, including procedures for


controlling access, providing personal protective equipment training employees
on safe work practices.
°As working as site inspector of the worksite, it is common practice to casually
ask supervision or guide on the worksite to talk to while evaluating potential
hazard factors. Mainly, site engineer or nonetheless the time keeper of the said
project which mainly secures the manpower safety.
These are the following control measure for hazards being identified:
PSYCHOLOGICAL HAZARDS
-Daily checking of individual motives or say connection or even the way they
communicate is a must and should be properly and intuitively practice by site
engineer or say foreman and any individual assigned to maintain working ethics
on their manpower.
EQUIPMENT FAILURES
-Any operator of any equipment heavy or not shall first inspect the working
condition of the said equipment and must do any necessary precaution in doing
so. Maintaining a good condition equipments saves time and lives does it can’t
be neglected throughout any project.

CHEMICAL HAZARDS
-Potential hazards from this factors only happen when there’s low level of
personal protective equipments specially during timely checking of equipment’s
condition. Does, well prepared and PPE is a must thing to do.
VENTIALTION HAZARDS
-While this is unlikely to happen, as any projects undergo massive evaluation
before granted. If possible threat appears so, PPE shall be used and must before
or while on instance of using equipments that creates fumes of harmful
substances.

BUILDING LOAD FAILURE


-While heavy equipments are working, it is good to check the load that an
undergoing building expansion can withstand as otherwise, it might collapse in
no time.
FIRE HAZARDS
-It’s quite common among newly constructed building or undergoing building to
easily be a potential fire to happen as materials used on such building are
exposed and naked does susceptible to fire specially to highly combustible
materials. Fire exit and well source of water shall be maintained at all time.
FALLING DEBRIS
-As usual at it is, complete PPE in a worksite helps workers away from
unnecessary fatal or accidents. Does, wearing hard hats and any PPE needed in
a construction of the project is a must.

3. Identify any monitoring and review procedures that will be implemented to


ensure that the control measures are effective and that the workplace remains
safe.
Planning or Making Objectives:
Most crucial part in monitoring nor implementing review procedure and or
implementing control measures.
It involves constructing of different objectives in accordance to the evaluated
hazard risks involve after inspection.
Devised Methods as per objectives
An objectives without proper method leads to nothing. Does, methods shall be
devised to ensure that the objective will be meet.
Introducing Policies that supports devised Objectives
A key to implement any procedure to follow in making control measures is to
accompany it with policies. Hence, by such on site engineers or individuals who
are task in humanitarian safety on the said project will ensure that the procedures
will be followed.
Make Reviews
Making reviews is one way to ensure that the implemented procedures are
followed. It is done by either monthly check or any period to identify any done
procedures or any other hazards exist. Since, changed is normal so does
hazards. Either it will be go out and another thing to exist. Does, Reviews plays a
great role.
Documentation
Documentation plays important role in implementation of procedures. While
these serves as proof, it also serves as basis to any under improve or
improvement to those identified hazards.

Along with these procedures, there’s ISO standards for every project. PAES is a
bunch of standards also that includes safety, assessments and standard
materials being used to a project ensuring present safety and onwards.

4. Write a brief report outlining your risk assessment and control measures. The
report should be clear, concise, and include relevant details and information.
PPE should always be observed at all times as project goes onwards. Most
probable hazard or most likely to happen is falling debris; which can be limit by
wearing hard hats. Other than that, securing good communication and
connection interaction to all worker shall also be observe well.
Daily checking of heavy equipments and maintenance will help reduced the risk
of equipment failures and risk. For an instance of fire hazards, workers shall be
inform the right fire exit. First aid kit shall always be present and perform
immediate response to any sort of accidents when workers are expose to cuts
and or falling objects. On chemical hazards, it important to separate high
combustible material from fire or any sort of equipment that can start a fire.
As per control measures and implementations; monthly of periodic checks shall
be done as project goes onwards while evaluation of new risk shall also be
assessed by the project inspector or site inspector. Documentation and proper
reviews of the project onwards shall be noted and inspection of control measures
shall assess by in every inspection. Moreover, any accidents and or risk shall
also be reported the site inspector to do proper and new control measures.
By such, project working ethics shall also be observed at all time. Periodically
visit is recommended to identify any risk that aren’t controlled yet and policies
shall be implemented if risk control measures aren’t observed or followed.
