AFFIDAVIT

I, MARC F. GELACIO, of legal age, Filipino citizen and a resident of

118 Roxas Street Burgos, San Marcelino, Zambales having been duly sworn
to in accordance with law, do hereby depose and say:

1. That I am in the process of completing my requirements for my

application for National TVET Trainer’s Certificate in DRIVING NC II
with the Technical Education and Skills Development Authority
(TESDA).

2. That I intend to submit the following documents to TESDA, to wit:

a. Letter of Intent/Application
b. Application form
c. National Certificate II in Driving NC II
d. Trainer’s Methodology Level I Certificate
e. Certificate of Employment from Gildean Engineering Services

3. That I hereby attest and affirm that the above-mentioned documents

that I will submit to TESDA are authentic, valid and true;

4. That I am executing this affidavit to attest the truth and the fact of the
foregoing statement and for whatever legal purpose it may serve.
