AFFIDAVIT OF DECLARATION

I, MA. TERESA R. NUQUI, of legal age, Filipino citizen and with residential address at
# 63 G. Felipe St., SFDM, Quezon City, after having been sworn to in accordance with law
hereby depose and say;

1. That I am the registered owner of the parcel of land and its newly built
improvements located at 61 G. Felipe St., SFDM, Quezon City;

2. That the actual building plan was made by Architect EDILBERTO F. GARCIA
and after all the necessary requirements facilitated, the construction of the
improvements was finally obtained a Building Permit No. 02-114404 on June 29,
2006;

3. That with the entire period of the construction, it was done through the
contractor ENGR. TEODORICO SIRINGAN who managed the construction and
of his direct supervision, said construction was covered by Construction
Agreement executed by herein affiant and contractor, duly notarized on March 7,
2007 by Atty. Romulo LL. Roy, a Notary Public for and in Q. C.;

4. That the actual construction was started sometime on March 2007 until it was
eventually finished on October 15, 2007;

5. That I am executing this affidavit in order to attest to the truth and the foregoing
circumstances and in support to the compliance with the government agencies
and for whatever legal purpose this may serve.

FURTHER, AFFIANT SAYS NOTHING.
