SURVEY QUESTIONNAIRE

This questionnaire aims to determine the current level of reading


proficiency of Grade 6 pupils of Dr. Domingo G. Abcede Elementary School, and
learn about their attitude towards reading, and the perceived factors affecting
their reading proficiency.

As a respondent of the survey, you are highly encouraged to answer the


items completely with honesty and sincerity.

Note: The surveyors/researchers will guide the respondents in answering each


item. Put a check (✓) on each item that corresponds to the respondent’s answer.
