About the state registration of

facilities in Pervomaisk

Pursuant to Article 25 of the Law of Ukraine "On Basic Principles and

Requirements for Food Safety and Quality", the Procedure for State Registration of
Facilities , Maintaining the State Register of Facilities of Market Operators and
Providing Information Therefrom to Stakeholders , approved by Order of the
Ministry of Agrarian Policy and Food of Ukraine No. 39 dated February 10, 2016 (
hereinafter - the Procedure), applications for state registration of facilities
capacity registration from 21.06.2018 LLC "Bandursky
oilextraction plant"

ORDERED:
1. To carry out on 22 .06 .2018 the state registration of the capacity of
Bandursky Oil Extraction Plant LLC (32390305 ) at 40 Tsentralna St., Bandurka
village, Pervomaiskyi district, Mykolaiv region, 55247, the type of activity planned
to be carried out using the capacity is the production of food products , the
ingredients of which are exclusively products of plant origin and /or processed
products of animal origin, and to assign the capacity a personal registration number
r-UA-14-18-796.
2. The Chief Specialist of the Food Safety and Veterinary Medicine
Department of the Pervomaisk Interdistrict Directorate of the Main Directorate of
the State Service of Ukraine for Food Safety and Consumer Protection in Mykolaiv
Oblast N.B. Kotelnyk shall , within three working days from the date of the
decision on state registration of the capacity , provide (send) the market operator
with an extract from this order and send the information to the Food Safety
Department of the Food Safety and Veterinary Medicine Department of the Main
Directorate of the State Service of Ukraine for Food Safety and Consumer
Protection in Mykolaiv Oblast by e-mail. region to the following e-mail address :
viddil_bhp@ukr.net.
 3. Control over the implementation of this Order shall be vested in the Head
of of the Pervomaiskyi Interdistrict Department of the Main Directorate of the of
the State Service of Ukraine for Food Safety and Consumer Protection in Mykolaiv
region, Glukhoy O.B.
