PERSONALIZED LEARNING PLAN PART B: TENNESSEE WORK-BASED LEARNING

Below is a list of skills that employers seek from their employees. Complete this section during the WBL
experience. Work with your teacher and/or employer to document the ways you practice these skills through your
experience. Also write down what evidence you can add to your portfolio after the experience to show your skills!

(You can learn more about what kinds of activities and learning opportunities are available at the workplace by
doing an internet search and interviewing the employer if that is possible. See the Pre- Experience Research
Checklist and Informational Interview Guide.)

APPLICATION OF ACADEMIC AND TECHNICAL KNOWLEDGE AND SKILLS

LITERACY: Read and comprehend relevant academic and technical texts


Goal: I will re-write the employee handbook adding in new modern pictures and checkpoints for
understanding.

My Experience:
● I read and learned the safety signs/labels to understand what to use and what not to use when cleaning.
● Reading work documents keeping private information secured.
● Reading specific documents from patient’s charts and scanning them.

My Evidence:

MATH: Select and apply relevant mathematical concepts to solve problems and perform expected tasks
Example: Close out the cash register by hand and compare to electronic results.

My Experience:
● Following a specific process step by step on how to document papers from patient’s charts.
● Having a fax machine to send documents quickly and securely.
● Answering the telephone when it rings at my designated desk.
● Creating digital folders to place certain files in.

● Sitting properly at a desk when scanning files.

UNDERSTANDING PATHS AND OPTIONS: Plan and navigate education and career paths
aligned with personal goals Example: Interview franchise supervisor about education needed;
document what is heard and analyze to student’s own plans.

My Experience:
● Working in a healthcare field will get me used to some of the same patterns I would see in my future
career path, becoming a Nurse Practitioner.
● Personal goal is to shadow or intern a doctor or nurse one day.
● Learning new healthcare terms and abbreviations that I can continue to remember and use in my
future career.

21ST CENTURY LEARNING AND INNOVATION SKILLS

CREATIVITY AND INNOVATION: Use imagination and insight to develop original ideas for products,
including physical products, services, and solutions to problems, among others
Example: Document participation in a brain-storming session and the ideas generated related to a new
marketing brochure.

My Experience:
● Building more office spaces and desks would allow more employees to have a place to put their clinical notes in
and to view their schedule daily.
● Adding more storage rooms would be beneficial.
● Building more operatories would help Cornerstone become more busy and to schedule more patients
throughout the day.

COMMUNICATION: Articulate ideas effectively in both oral and written communications; listen effectively
Example: Orally present the results of a survey of students about their interest in a new app.

My Experience:
● Problem-solving efficiently with my peers or manager.
● Showing effective listening skills by scanning and documenting charts accurately.
● Speaking clearly and professionally when answering the phone.

My Evidence:

INFORMATION LITERACY: Access and evaluate Information, manage information accurately and
ethically Example: Conduct an internet search about competitors in the youth clothing industry in the
community, documenting sources and rating each for credibility.

My Experience:
● When scanning and charting patient’s information, I use HIPPA and am aware to not share their information and
to keep it private.
● Follow confidentiality rules and keep their information safe and not discuss it outside of my workplace.

PERSONAL AND SOCIAL SKILLS

INITIATIVE AND SELF-DIRECTION: Work independently; demonstrate agency, curiosity, and the
ability to learn Example: Take the initiative to find out more about the science behind a process at the
manufacturing plant and write up what was learned.

My Experience:
● Work without supervision, collecting charts on my own, cleaning up after myself, and completing all
tasks by the end of my shift without additional assistance.
● Take the initiative to learn more about the different methods used to process and document all the
patient's charts.

My Evidence:

CULTURAL AND GLOBAL COMPETENCE: Exhibit interpersonal and social skills that are respectful
of cultural differences Example: Identify staff of differing cultural origins and document conversations
about cultural differences in expected workplace behavior.

My Experience:
● Front office is decorated for most of the religious holidays in the lobby.
● They hire employees who are different races and have different religions.

PRODUCTIVITY AND ACCOUNTABILITY: Set goals and priorities and manage time and projects;
exhibit punctuality, persistence, and precision and accuracy; complete projects to agreed-upon
standards Example: Verify (and document verification of) the sums on a spreadsheet of donations before
turning it in on time.

My Experience:
● Managing time by making a calendar with a checklist to follow.
● Completing all assignments on time with precision.
● Prioritizing my tasks by sorting documents accurately in a way to make them easy to scan, so my
time is used efficiently.
● Showing up on time when scheduled and trying my best on each task.
