gastroenteritis, infection ni sya, kani na tambal makatabang ni para mupatay sa

Nursing Considerations

 Watch for seizures; notify physician immediately if patient develops or

increases seizure activity.

 Monitor signs of pseudomembranous colitis, including diarrhea, abdominal

pain, fever, pus or mucus in stools, and other severe or prolonged GI
problems (nausea, vomiting, heartburn). Notify physician or nursing staff
immediately of these signs.

 Monitor signs of allergic reactions and anaphylaxis, including pulmonary

symptoms (tightness in the throat and chest, wheezing, cough dyspnea) or
skin reactions (rash, pruritus, urticaria). Notify physician or nursing staff
immediately if these reactions occur.
KETOROLAC 30mg IVVT now then PRN for PS greater than or equal to 5

gihatag ni mam kay kaning tambala gina parelieve ang pain nga nasinati nimo. I
administer ni sya mam pinaagi sa IV. (Pain scale : 6/10, 9/10) if with activity.

Naa ni syay mga possible side effects mam like paninguton ka, kalipong, kasukaon,
labad ang ulo, ug katugon sad ka ani, dili lang ta kaayo mag lihok2 mam, higda lang
gyud ta, if ma feel to nimo nga possible side effects mam, i notify dayun ko or imong

NR:

 Don’t forget to assess first the patient before administering this drug: know the
history (e.g. allergies, renal impairment, etc.) and physical condition of the
patient (reflexes, ophthalmologic and audiometric evaluation, orientation,
clotting times, serum electrolytes, etc.)

 Watch for S&S of GI ulceration and bleeding (e.g., bloody emesis, black tarry
stools) during long-term therapy.
 Note: Possible CNS adverse effects (e.g., light-headedness, dizziness,
drowsiness).
OMEPRAZOLE 40mg IVTT now then OD

Gihatag ni na medication ma’am based saimong condition at risk ka for impaired

swallowing, Kaning omeprazole gihatag para maprevent ang NSAID- induced
ulcers , stress ulcer prophylaxis in critically ill patients.

Side Effect: pag human nakog administer ani sa imo mam/sir di jud nato maiwasan
nga basig makasinati kag mga sintomas sama sa Sakit sa ulo, pagkalipong,
kakapoy, kalibanga, sakit sa tiyan, kasukaon, kung makasinati man pinduta ang
nurse call button dira sa gilid aron ma inform ko og i-assess tika, og mahatagan kag
proper interventions mam.

NR:

Monitor other CNS side effects (drowsiness, fatigue, weakness, headache), and
report severe or prolonged effects.

Advise patient to avoid alcohol and foods that may cause an increase in GI irritation.

Instruct patient to report bothersome or prolonged side effects, including skin

problems (itching, rash) or GI effects .

IVIG 28mg IVTT now then OD for 4days

Gi prescribed saimo ni doc ma’am since naa ky GBS. Kanin a medication is

ginagamit for treating autoimmune diseases. Ihatag ni siya intravenously ,
pinaagi sa vein. Kani na medication tabangan imong body na magfight sa
infection.

SE: headache, flushing, chills, myalgia, wheezing, tachycardia, lower back pain,
nausea and hypotension.

NR:

 Make sure emergency drugs and appropriate emergency facilities are

immediately available for treatment of anaphylaxis or sensitization.
  Note: Hypersensitivity reactions are most likely in patients receiving rapid IV
infusion.
 Monitor vital signs and infusion rate closely when patient is receiving IGIV.
 Note: IGIV has a mild diuretic effect in some patients due to presence of
maltose.
 Report immediately S&S of hypersensitivity.
 Report immediately infusion symptoms of nausea, chills, headache, and chest
tightness; these are indications to slow rate of infusion.

NIFEDIPINE 30mg 1tab PO OD

admit nimo dinhia, taas imong blood pressure (140/100mmHg), kani nga med mam
gina pababa niya imong blood pressure nga gipa na relax niya ang imong ugat kung
asa naga agi atong dugo para si heart dili na sya mag pump ug todo todo. Inumon ni
sya mam ka isa sa isa ka adlaw pinaagi sa baba

Naa ni syay mga possible side effects mam like kainit sa lawas, naga kurog2 atong
lawas, kalipong, ug labad ang ulo, dili lang ta kaayo mag lihok2 mam, higda lang
gyud ta, if ma feel to nimo nga possible side effects mam, i notify dayun ko or imong
kauban mam ha para mahatagan dayun nato ug interventions.

NR:

 Monitor patient carefully (BP, cardiac rhythm, and output) while drug is being
adjusted to therapeutic dose; the dosage may be increased more rapidly in
hospitalized patients under close supervision. Do not exceed 30 mg/dose
increases.
 Ensure that patients do not chew or divide sustained-release tablets.
 Taper dosage of beta-blockers before nifedipine therapy.
 Protect drug from light and moisture.
 Do not chew, cut, or crush sustained-release tablets. Swallow whole.
 You may experience these side effects: Nausea, vomiting (eat frequent small
meals); dizziness, light-headedness, vertigo (avoid driving and operating
dangerous machinery; take special precautions to avoid falling); muscle
cramps, joint stiffness, sweating, sexual difficulties (reversible).
  Report irregular heartbeat, shortness of breath, swelling of the hands or feet,
pronounced dizziness, constipation.

CAPTOPRIL 12.5MG 1tba SL PRN for elevated BP

Kaning Captopril Ma'am antihypertensive drug ni sya maam buot pasabot akoa
syang ihatag sa imoha para mobaba ang atuang BP. Maam sublingual ni sya maam
ha buot pasabot ibutang lang ni sa ilalom sa dila ug palanayon lang nato maam ha
dili lang nato tunlon. Sublingual ni sya maam kay daghan ug blood vessels (ugat)
ilalom satong dila maam, mas paspas ang pag sulod sa tambal satong lawas.

Naa ni syay mga possible side effects mam like kalipong, maka feel sab ka na mura
kag ubuhon, if ma feel to nimo nga possible side effects mam, i notify dayun ko or
imong kauban mam ha para mahatagan dayun nato ug interventions.

NR:

Monitor BP and pulse frequently during initial dose adjustment and periodically
during therapy. Notify health care professional of significant changes.
PREDNISONE 20mg 1tab PO OD

VITAMIN B COMPLEX 1tab PO OD

ni sya sa brain para mag function well ug naga promote pud ni sya sa atong cell
growth ug sa pag produce sa atong RBC. Inumon ni sya kaisa sa isa ka adlaw
pinaagi sa baba.

Naa ni syay mga possible side effects mam like manlabo imong mga mata, uhawon
kag maayo, ug sakit ang tiyan, if ma feel to nimo nga possible side effects mam, i
notify dayun ko or imong kauban mam ha para mahatagan dayun nato ug
interventions.

PRAZOSIN 500mcg 1tab PO BID

pressure pinaagi sa pag relax niya sa imong mga ugat kung asa mu agi atong dugo,
ug na relax na kani, mas dali na mu daloy atoang dugo.

Naa ni syay mga possible side effects mam like labad sa ulo, kasukaon, kalipong,
katugon, manlabo imong mga mata. if ma feel to nimo nga possible side effects
mam, i notify dayun ko or imong kauban mam ha para mahatagan dayun nato ug
interventions.

MYCOPHENOLATE mofetil 1gm tab PO OD

Mycophenolate mofetil (MMF) is a new immunosuppressant recently

introduced in the treatment of autoimmune conditions.
Gihatag ni doc, since naa kay GBS which autoimmune condtion. Ginahold back ang
immune system and nagatabang para maprevent ang cell damage ug inflammation.

SE: Stomach pain, nausea, vomiting, diarrhea, constipation; swelling in your ankles
or feet; rash; headache, dizziness, tremors; fever, sore throat, cold symptoms, or
other signs of infections

NR:

Be alert for signs of increased white blood cell counts (leukocytosis), including fever,
weakness, weight loss, loss of appetite, dizziness, fainting, dyspnea,
bleeding/bruising, or pain or tingling in the arms, legs, or abdomen. Report these
signs to the physician or nursing staff immediately.

Switch from PREDNISONE to METHYLPREDNISOLONE 16mg 1tab PO OD
