INCIDENT REPORT

NARRATIVE: On Monday, March 27, 2023, while Mr. Pilar was teaching Grade 10 Mathematics, his
colleague Mrs. De Guzman abruptly cut off their dicussion and accused him of the following: (1) Mr. Pilar
had closed the door and was refusing to let latecomers into his class. (2) The students' emotions have
been expressed using the phrase "NATATAKOT SILA SAYO." The incident occurred in front of his
students, according to Mr. Pilar, who felt bad about it. The group that included Jizza Balisalisa and
Frances Mary identified Maria Fe Avelino when Mr. Pilar questioned the students about whether or not
it was true that they were terrified of him. Mr. Pilar made the decision to present the matter to the
guidance office in front of me, the school's guidance counselor, in order to ascertain whether Mrs. De
Guzman's claims that they are afraid of him are accurate. However, Ma. Fe denied using these phrases
to characterize the circumstances, "Wala po akong sinasabing natatakot kami kay sir, nadaanan lang po
kami ni ma'am Virgie sa CR at sinabi namin sa kanya nasaraduhan kami ng pinto sa klase ni Sir," The
students continued to deny using the word "NATATAKOT" when Sir Elton inquired about it again, saying
that it was utterly out of context. Later on, Mrs. De Guzman asserted that the scenario in which the
group of Ma. Fe is outside and is not asking Mr. Pilar's permission to enter the classroom led her to draw
the conclusion and claim that these students are terrified of him. She also claimed that her claim was
based on her emotions, which is untrue based on the students' statements.

This issue has been reported to me this 27 th day of March 2023 and is resolved on the same day.

Furthermore, this issue is resolved and will not be used against any of the persons involved.
