Parental Permission Slip for

City Museum Field Trip
On Friday, May 5, 2023, Field Day for the secondary
students will be held at the City Museum. This will be a
free event for the students that return this permission slip by
Monday, April 24th, otherwise the price is $15.00. The
students will be transported from NCCS to the museum
by bus.

The planned itinerary is as follows:

Departing from the school: 9:30 am

Arriving back to school: 12:30 pm

Please complete the permission form below and

return it to Mrs. Ewing by noon, Monday, April 24 th.

to go to the City Museum on May 5, 2023. I am
aware of the student fee of $15.00 must be paid if this
form is not returned to the school by noon on Monday,
April 24. I am also aware of the departure/return times
and the method of transportation.
