Student’s Exit Slip

HARANA: Himig ng Awiting Pag-ibig is a Music Festival that brings out the community’s harmony
by gathering students and staff on a night filled with music and entertainment. This event will be held at the Quezon
City Science High School Football Field from 3:00 PM until 9:00 PM.

On the day of the event, no vehicles will be permitted to enter and park inside the school premises. No cars
will be allowed to pass through Golden Acres Road, so students must be dropped off near the DepEd NCR Regional
Office and fetched at the 2nd Gate in front of The Grass Residences.

In an effort to protect and provide maximum safety for our participants, we ask you to fill out this form to
let us know how the said student will go home after the event. Students without an exit slip will not be
permitted to leave campus without a fetcher.
