FIELD TRIP PERMISSION SLIP

Dear Parents,

As a part of our study of Art Appreciation and History, we will be taking an educational tour to
the famous museums and heritage sites located in Cebu City. We will leave Ormoc City on May
18, 2022 at 10:00 pm, and we will take a ferry together to Cebu. Students will have an
opportunity to see old Philippine culture as well as architecture. A series of lectures will also be
included during the tour so this trip will be very informative for the students.

LETTER OF INDEMNITY

I, _____________________________, father/mother/guardian of ________________________,

a 3rd year Political Science student, voluntarily permit my child to go for the field trip on 19th
May, 2023 – 21st May, 2023.

I understand that the school and the course instructor have taken great care in planning this trip
and the safety of each child is of utmost importance in the school.

As a parent, I have given accurate information regarding the blood group of my child, allergies,
medication, special conditions and special care needed in case of any emergency. I understand
that the school will take all the necessary actions to secure the safety of the child throughout the
trip. However, in case of any unforeseen circumstances beyond the control of the school and
the instructors, I will not hold the school nor the instructors responsible.
