Investigation Narrative:

There was no failure on the part of the AV bay relating either to process or database control, they
were given the wrong computer to test. The lengthy delay in supplying the computer for test and the
fact that the UFCM Work Order was coordinated having identified a different computer (#833) as
being the removed component would tend to confirm this. Although the SNOW on the handwritten
F731 attached to computer #881 is 0907ZJ956041007, the date on the Condition and Safety
Certificate on the same form is 18 Dec 07, indicating a delay of nearly 11 weeks. Computer #881
has since been retested and will be returned serviceable to stores.

Proposed Corrective / Preventive Action:

The delay between identifying a computer as a possible cause of a UFCM (or any other fault) and
passing it on to the AV bay for testing should be kept to a minimum. Ideally, the computer should be
sent for test without being placed in storage for any length of time, avoiding the possibility of sending
the wrong one to the bay.
