MED AD PREPARATION
Follow what’s in the checklist kay
● Prepare medication ticket (5)
● Prepare vial, ampule, syrup, and
tablet
● Prepare the said syringes and
needles used (5 syringes will be
used for preparation; 2 for final
use)
ORAL (TABLET)
1. Check patient’s chart
2. Compare medication with
medication ticket
3. Check doctor’s order
4. Compare (name, dosage)
5. Check MAR
6. Prepare medication with
medication ticket on tray.
PROCEDURE:
1. Put the medication tablet in the
med cup (unpopened) underneath
is the med ticket.
SYRUP
1. Hold the bottle in such way that
you’re covering the label (to avoid
droplets on the label)
2. Pour syrup on the eye level
3. Put medication on tray with med
ticket underneath
VIAL
1. Read amount of diluent needed
2. Prepare sterile water to be used
3. Open vial (use bandage scissor;
rubber cover is to be opened using
the tip of scissor whilst to
completely remove, use the “ubos
near the handle nga part then i clip

4. Get syringe
5. Change needle
6. Get sterile water needed
7. Always aspirate for air

AMPULE
1. Prepare syringe

STERILE

ORAL
1. Introduce self
2. Check identifiers (name, birthday)
3. Tell patient the need to take
medication
- Oral (fever, cough, etc.)
- ID (check allergen reaction to
medication ordered by doctor)
- IM (administer medication
ordered by doctor)
4. Ask patient how he/she prefers to
drink tablet (by palm of hand or
med cup)
5. Let patient drink water
6. Check mouth by moving tongue
up, down, left, right to ensure
medication had been taken
SYRUP
1. Continuation

1. Measure tube (start from nose, ear,

5. Insert asepto syringe (bulb is
attached already)
6. Prepare stet and i put sa may
stomach sa patient
7. Push bulb syringe for air then
steady then listen for gurgling or
swishing sound
8. Remove bulb
9. Clamp tube
10. Remove stet
11. Pour water for flushing (do not
unclamp until water has filled the
tube)
12. Unclamp then elevate 6 inches
from the stomach
