sphygmomanometer.

Good afternoon ma’am I’m Eileen Valino, your student


nurse from Good Samaritan Colleges.So ma’am may I ask
what is your name?When is your birthday ma’am? So for
today ma’am I will be taking your temperature,
respiratory rate, blood pressure and pulse rate to check
if there is any problem with your health or not at all.

This vital sign assessment will take 10-20 minutes.

Perform hand hygiene.

I will close the curtain to ensure the patients privacy.

Ask for pain scale 0 to 10.


Ask for her normal BP.

1.Place the thermometer; remove and read temperature.


2.Take pulse rate; palpate and count pulse accordingly.
3.Take respiratory rate; observe and count the RR.
4.Take Blood pressure; auscultate the clients BP.

Perform hand hygiene.

Document and report findings.

Inform the patient for vital sign results and ask if


there’s a question.

Thank the patient for cooperation and time.
