Ask for pain scale 0 to 10.
Ask for her normal BP.
1.Place the thermometer; remove and read temperature.
2.Take pulse rate; palpate and count pulse accordingly. 3.Take respiratory rate; observe and count the RR. 4.Take Blood pressure; auscultate the clients BP.
Perform hand hygiene.
Document and report findings.
