This Evaluation Sheet has two parts, Part A and Part B. In Part A, you need to enter specific tasks which
you have given to your team member for the time period applicable for the evaluation. The total weight
of all the tasks in Part A should be tallied to 100.
Part B consists of general traits that employees are expected to demonstrate. The HOD/immediate
supervisor need to assess the employee by selecting the rating applicable to achievement of task or
trait.
For the final summary, 60% of the score will come from Part A and 40% of the score will come from Part
B.
The AGM - Administration will moderate the Ratings given.
In addition to the ratings, the employee, HOD and AGM - Admin can provide qualitative feedback in the
areas given.
