SIRE INSPECTION PROCESS

Inspection Routine (Subject to Master's discretion)


Total Time for a standard inspection without delays 08 - 10 Hrs

Following documents are requested to be kept ready for sighting at location


indicated (Only the ones applicable)
Master’s Office
• Crew List, Ship’s particulars
• Vessel’s trading/statutory certificates including DOC, SMC, and certificate of class,
ISPS, CSR, NLS, MLC, SEEMP and SECA approvals
• Non-statutory safety and operational certificates (LSA/FFA, Bow stopper certificate
from class, VDR, EPIRB, SART, AIS, Shore based maintenance, Chart Supply contract,
lifeboat on load release annual and periodic, Means of embarkation periodic etc)
• Mooring equipment file (Positive identification of wires and ropes, certificates, usage
and maintenance. Winch break test certificate. Lifting gear register
• Last Port State control inspection report
• Officer’s and crew National certificates, DCE, GMDSS, Flag state certificates,
endorsements and training records for ECDIS, BRM, SAFETY/SECURITY OFFICER.
• ISM manuals – Navigation – safety – cargo – emergency procedures and Policies
• Latest class status report / conditions and memoranda to note
• Enhanced survey file – Thickness measurements, CAP ratings, CAS, Condition evaluation
report
• Tank inspection records: Ships staff and class
• Approved manuals Trim & Stability data, damage stability information, Loading
Manual/P & A Manual and cargo record book (If Applicable)
• Approved manuals for IG, COW, ODME & load Computer (Class Certificate and test)
• Oil record book Part 1 (Engine), Part 2 (Deck), Manual for approved OWS
• SOPEP / SMPEP & VRP, ERS or ETAS Contract
• Garbage record book and Garbage management plan
• Hot-work, enclosed space entry permits /other work permits /Risk Assessments file.
• Ballast water management plan and records of ballast transfers
• Ship specific SOLAS training / Fire training manual and fire safety operation booklet
/ LSA FFA ship specific instruction and maintenance Manual as per SOLAS requirement
• Familiarization checklists, hours of work and rest (All departments)
• Safety committee meeting minutes / Near misses / Accident reports and closeout
reports of all from company.
• On board Drill records – LSA – FFA – Steering – Antipollution – emergency
procedures
• Superintendent visit reports / Internal audit reports, follow-up action and closeout.
• Master’s review of SMS and closeout by company.
SIRE INSPECTION PROCESS

• D&A policy and certificates of testing for internal Alcohol and external drug and
Alcohol
• Test record of safety valves and calibration for temperature and pressure gauges
(GAS)
• Ship security records (last 10 ports, training and drills, changes in security level,
security threats and incidents, audits and reviews of security plan, officer training)
• Technical Files NOX certification for main engine and generators (EIAPP)
• Bow mooring diagram showing SBM operation

Bridge (Please keep all documents below on Bridge only)


• Bridge procedures Manual (Company specific)
• Various Log Books (Deck Log, Maneuvering log, Compass error, Deviation Card, GPS,
GMDSS, Radar, Celestial observation, Bridge order book, Emergency battery)
• Standing Instructions
• Navigation Checklists, deck log book, bell book.
• Voyage Charts with last voyage courses marked, statutory publications and chart logs
and Passage Plan

Cargo Control Room (Please keep all documents below in CCR only)
• Cargo Plan / COW plan and execution
• Pump purging records (If applicable)
• Ship/Shore safety checklists
• Portable and fixed instruments Log and instrument testing procedures
• IG and COW Log
• Chief Officers standing instructions and Order Book
• Maintenance records or PMS for IG non return valves, PV valves and flame screens
• Cargo Log Book (Port Log)

Engine Room (Please keep all documents below in Engine Control Room only)
• PMS records including critical spares list
• Dates of last test of critical machinery
• Standing instructions and order book
• Engine Log Book
• Alarm Log, Engineer alarm test records
• Bunkering records including BDR, MSDS and Checklists
• Lub-oil Analysis
• Bunkering records including BDR and Checklists
